Master's Degree from accredited school of Social Work or related field; Valid license/registration of permit to practice in state(s) of agency operation if required

Experience or education in grief counseling preferred.

Responsibilities:
As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

Completes individual plan of cares with patients, family/care giver and care team members.

JOB SUMMARY:
The Behavioral Health Specialist will be responsible for providing episodic behavioral health and clinical services to a wide range of individuals and families including but not limited to foster children, their biological families, foster families and/or adoptive families as well as other children, adolescents, adults and families covered by other insurance plans/payers as assigned.
MAJOR DUTIES & RESPONSIBILITIES:
Complete initial diagnostic assessments on all patients assigned to determine treatment needs, as well as conduct ongoing assessments to determine when clinical goals have been achieved and service delivery is no longer indicated;
Develop and implement initial treatment plans and update treatment plans to include age appropriate clinical interventions/objectives/goals that are observable and measurable;
Provide support and crisis management services as needed or as requested by supervisor;

MFLC Counselors provide assessment and short-term, solution-focused counseling services to eligible individuals, marital or intimate partners, families, and groups.
Ability to perform accurate assessment of the client's presenting problem and to plan effective treatment for that problem. Knowledge of preventative models that strengthen service members' and their families' readiness and resilience by reducing the stresses/stressors of military life cycle.
MFLCs have extensive knowledge of the military chain of command and feel comfortable interacting with all ranks within a military structure.

The Detention Counselor is a uniformed, security trained member of the Unit Management Team responsible for resolving daily inmate issues before they become significant matters, incidents or grievances. They will ensure that services and programs are delivered to inmates assigned to the unit at a time and manner as designed.
* Maintain a caseload of inmates with the primary purpose of resolving daily living issues before they expand into significant incidents.
* Make daily rounds through assigned units talking with staff and inmates, observing operations and interactions between staff and inmates on the unit, ensuring the unit is operating in a safe and secure manner and anticipates situations.
* Conduct one-on-one meetings with inmates regarding grievance or potential grievance matters and resolve the situation within the framework of policy.
* Develop an interactive working relationship with inmates that support the informal resolution of inmate issues.
* Develop and maintain a cooperative working relationship with institution departments built on respect that supports problem solving.
Qualifications:
* High school diploma, GED certification or equivalent.
* Two years of security experience in a criminal justice field, which preferably includes one year in a supervisory capacity.
* Additional education may be substituted for the experience on a year-for-year basis.
* A valid driver's license is required.
* Minimum age requirement: Must be at least 18 years of age.

/RESPONSIBILITIES
Provides professional level skills required in the clinical practice of mental health counseling and crisis intervention, affording direct services to individuals housed in the adult detention center. Screens and identifies inmates who are mentally ill, have substance use issues, and/or may be at risk for suicide and make the appropriate referral for further assessment. Works as a therapeutic agent as a member of a multidisciplinary team to provide evaluation, treatment planning and service coordination for those in adult detention. Counsels and assists patients with personal and environmental difficulties which predispose illness or hinder the patient from receiving maximum benefits from medical care. Actively supports the policies and procedures of the University Health.
EDUCATION/EXPERIENCE
Master's degree in Social Work from a school accredited by the Council on Social Work Education is required. One year post master's degree experience in a health care setting is preferred.
LICENSURE
Current state licensure from the Texas State Board of Social Workers Examiners is required. Current CPR certification as a BLS HealthCare Provider is also required. Certification as a Correctional Health Care Professional is preferred when eligible.
