Position Overview: As a Permanency Case Manager, you will promote the achievement of permanency for children in the Managing Conservatorship of DFPS by providing services to a caseload of children and families by working in partnership with the family. The focus is to assess children and family's needs, safety issues, and services identified mutually with the family to allow the family to safely assume the role of parenting their children. For those families where that is not possible, the focus is on achieving permanency for the child in the most family-like setting.
Key Responsibilities:
Manage a caseload of Permanency placements, supporting children and families throughout the process.
Assess placement needs, safety, family strengths, and placement resources for children and caregivers.
Facilitate permanency planning, ensuring a safe and nurturing environment for children in care.
Continuously search for and evaluate potential Kinship placements.
Conduct monthly (or more frequent) visits with children and families based on their needs.
Provide ongoing support services post-reunification until court case dismissal.
Collaborate with public and private agencies to coordinate and deliver Permanency services.
Conduct assessments, complete court reports, and provide testimony in court hearings.
Ensure the accuracy and completeness of case records for children and families.
Facilitate family preparation for permanency through training, mentoring, assessments, and home visits.
Work with the Adoption Coordinator to prepare children for adoption placements.
Provide 24/7 on-call support for children and families, including crisis intervention when needed.
Collaborate with Care Management staff to ensure timely and appropriate placements and with PAL staff for older youth's Transitional Living services.
Compile and report monthly statistical data to relevant agencies.
Promote cultural diversity and sensitivity in all interactions and work-related activities.
Drive personal or agency vehicles for transportation of children and agency-related tasks, adhering to agency standards.

What we are looking for:
Current, valid, and active license to practice as a Registered Nurse in the state of employment required.

Maintain a regular schedule of contacts and case management services on the assigned caseload in accordance with program guidelines and the needs of each case. Goals and methods related to these contacts are to be established in consultation with the Children's Program Administrator, parents/managing conservators and, when possible, with the residents who we serve.
Assess, coordinate, and refer residents for appropriate services such as psychological evaluations, counseling services, and other therapeutic services as needed and within the time frames.
Act as medical consenter for residents and monitor resident's medical, dental, therapy visits, and track all visits to meet DFPS time requirements. They provide person directed skills training and psychosocial rehab to develop coping and other skills needed to feel successful in their home and community. This position is responsible for initiating and collaboratively completing uniform assessments then organizing and documenting the recovery plan for services to meet the individual's identified needs and is effective at troubleshooting and organizing support, interventions, and services to achieve outcomes. The Intake Coordinator / Counselor coordinates, monitors, provides recovery planning, serves as an advocate and liaison on behalf of the individual served.

Direct service hours anticipated per month are no less than 50% of work time and meet utilization management standards by level of care. At least 1 skills training and/or psychosocial rehabilitation group service is provided each shift. 90% of full chart reviews receive a score of 80 or higher in following the Golden Thread. All required trainings are completed prior to due date. Attends monthly team meeting.
Due to the Crisis Center operating 24/7, employees may be required to work on center holidays if the holiday falls on their scheduled workday. In the event of unforeseen circumstances that require the use of Paid Time Off, employees are expected to notify their supervisor within 4 hours of their regularly scheduled work time. This position requires working three 12-hour shifts per week. Position Overview: As a Permanency Case Manager, you will promote the achievement of permanency for children in the Managing Conservatorship of DFPS by providing services to a caseload of children and families by working in partnership with the family. The focus is to assess children and family's needs, safety issues, and services identified mutually with the family to allow the family to safely assume the role of parenting their children. For those families where that is not possible, the focus is on achieving permanency for the child in the most family-like setting.
Key Responsibilities:
Manage a caseload of Permanency placements, supporting children and families throughout the process.
Assess placement needs, safety, family strengths, and placement resources for children and caregivers.
Facilitate permanency planning, ensuring a safe and nurturing environment for children in care.
Continuously search for and evaluate potential Kinship placements.
Conduct monthly (or more frequent) visits with children and families based on their needs.
Provide ongoing support services post-reunification until court case dismissal.
Collaborate with public and private agencies to coordinate and deliver Permanency services.
Conduct assessments, complete court reports, and provide testimony in court hearings.
Ensure the accuracy and completeness of case records for children and families.
Facilitate family preparation for permanency through training, mentoring, assessments, and home visits.
Work with the Adoption Coordinator to prepare children for adoption placements.
Provide 24/7 on-call support for children and families, including crisis intervention when needed.
Collaborate with Care Management staff to ensure timely and appropriate placements and with PAL staff for older youth's Transitional Living services.
Compile and report monthly statistical data to relevant agencies.
Promote cultural diversity and sensitivity in all interactions and work-related activities.
Drive personal or agency vehicles for transportation of children and agency-related tasks, adhering to agency standards.

Minimum Qualifications

* be at least 21 years of age;
* have no disqualifying criminal history as described in the TAC;
* never have had any type of certification revoked by the Texas Juvenile Justice Department;
* complete the training required by the TAC; and
* pass the certification exam as required by §344.700 of the TAC or any successor statute.
Certification as a Juvenile Supervision Officer by the Texas Juvenile Justice Department within
six (6) months of initial employment is required.
Possession of a valid driver's license. Must be able to obtain a State of Texas driver's license within 90 days.
