The Aftercare Case Manager provides ongoing support and coordination for youth and families as they transition from residential treatment to community life. This role ensures continuity of services, connects families to needed resources, and helps youth continue their healing journey through structured aftercare planning. The Case Manager collaborates with the Service Planning team, assists with school enrollment, medical and mental health provider transitions, and tracks aftercare outcomes to promote long-term stability and success. This role is based out of Abilene.
Key Responsibilities
Collaborate with the Service Planning team to create and monitor individualized aftercare plans.
Manage a caseload of approximately 25 youth receiving aftercare services.
Conduct and participate in aftercare check-ins to assess progress and needs.
Assist with school enrollment and maintain educational portfolios.
Coordinate medical and mental health service transitions to ensure no lapse in care.
Document all services and outcomes accurately and timely in accordance with agency and state standards.
Support parents and caregivers in accessing community resources and normative, age-appropriate activities.
Adhere to all New Horizons policies, state, and federal child care regulations.
Maintain a commitment to New Horizons' philosophy of Keeping Kids First in all actions and decisions.
Qualifications
Bachelor's degree and two years of experience working with youth who have experienced trauma preferred; or a master's degree in a social service or related field.
Valid driver's license, proof of insurance, and ability to transport youth as needed.
Strong organizational, communication, and documentation skills.
Ability to build positive relationships and work collaboratively across disciplines.
Understanding cultural diversity and the ability to work effectively with diverse populations.
Must pass a criminal background check, drug screen, and tuberculosis screening.

The School of Social Work at Abilene Christian University invites applications and/or nominations for a tenure-track Assistant, Associate, or Full Professor in the Master of Science in Social Work program. This is a full-time, 9-month, tenure-track faculty position. The primary work location will be on the residential Abilene Campus. Initial salary is commensurate with qualifications, experience, and educational degree. ACU's School of Social Work offers both the Bachelor of Science in Social Work (BSSW) and the Master of Science in Social Work (MSSW) degrees. Although the successful candidate will be primarily assigned to the graduate program, they will be expected to teach at both the graduate and undergraduate levels. The graduate program has a single specialization in Advanced Generalist Practice.

Qualifications
Applicants must hold a Master's degree in Social Work from a CSWE-accredited or internationally-recognized social work program. An earned doctorate is expected with strong consideration of ABD candidates in Social Work or a closely related field. Experience in university teaching and evidence of scholarship or scholarly potential is preferred. Teaching experience in an online modality is highly desirable.
The successful candidate must have at least two (2) years' post-MSSW practice experience. The ability to teach across the social work curriculum is expected. However, preference will be given to candidates who have strong clinical/micro-practice experience, particularly in integrated health and behavioral health. Possession of the LCSW or equivalent is highly desired.

Education, Training, and Experience
Must hold at least a high school diploma (an associate degree or more advanced degree preferred) and have current full licensure as a Licensed Chemical Dependency Counselor, Licensed Professional Counselor (LPC) or LPC Intern, Licensed Marriage and Family Therapist (LMFT) or LMFT Associate, or Licensed Clinical Social Worker by the Texas Department of State Health Services. Skilled in the use of personal and network computers, word processing, database, spreadsheet, and office management software (Microsoft Office), familiarity with CMBHS and/or Smartcare is a plus.
Ability to abide by the Centers' and the specific SAS Department's policies and procedures. Demonstration of knowledge of the Texas Administrative Codes specific to delivery of substance abuse services and the ability to abide by the guidelines set forth in the Texas Administrative Code.
