Job DescriptionThe CaseManager is responsible for intake, admissions, staffing, discharge screening, and documentation for children and youth in care. All New Horizons Staff are expected to study, practice, and operate within the New Horizons' Philosophy and Treatment Model.

Duties and responsibilities
Keeping Kids Safe!
Following the New Horizons' Employee Handbook;
Work collaboratively with other personnel, service providers, professionals, and biological/kinship/foster families involved with the youth on their caseload;
Managing an average of 15-20 youth (service level dependent) for children in foster care while adhering to all State Minimum Standards and New Horizons' policies;
Demonstrating flexibility and adaptability to best meet the needs of kids, and able to respond 24/7 to crises on a rotating on-call schedule;
Cross-system coordination, including maintaining and supporting the child's school, medical, dental, behavioral health, PAL, and other service needs. Must be well-versed in STAR Health services to ensure that youth at each service package level receive relevant support;
Ensure that the educational portfolio remains updated; and all school needs including enrollment, academic performance, etc. are met
Conducting and participating in intake or assessment interviews;
Accurately completing all required documentation in a timely manner;
Attending and actively participating in meetings related to the kids in your caseload;
Ensuring all psychological, medical, dental, and vision updates are current;
Reviewing all therapeutic plans with the New Horizons Treatment Director;
Ensuring that staff are adhering to New Horizons', State, and federal regulations of childcare;
Attending and completing all required professional training and workshops to satisfy continuing education hours;
Transporting kids in a personal vehicle when needed; and
Studying, practicing, and operating within the New Horizons' Philosophy of Keeping Kids First.
Qualifications
A bachelor's degree and two years of relevant experience preferred; or
Minimum requirements may be substituted with a master's degree in a social service or related field;
Educational or experiential qualifications will be considered equivalent.
Maintain a valid driver's license and insurance;
Must have insight and understanding of cultural diversity and have the ability to work with diverse populations;
Must be able to perform conflict resolution or crisis intervention;
Must pass a criminal background check, drug screen, and tuberculosis screening.
Demonstrate effective oral and written communication skills, as well as excellent organizational skills;
Demonstrate the ability to fulfill the technological requirements and the position, including a working knowledge of Windows operating system to the most current edition, Microsoft Word and Excel, email and electronic communications, and effective Internet browsing in regards to information gathering, research and staying up-to-date on social norms, current events or other information that could affect the children in care, their treatment or the Case Manger's relationship with them;
Possess extensive prior experience that demonstrates their cognitive ability to assist the child in difficult, crisis, and demanding times; and
Work under the direct supervision of the Child Placement Management Staff with a wide degree of discretion and independent judgment.
Working conditions
Working in a mixed indoor and outdoor environment, involving continuous interaction with children, requires flexible hours.
Physical requirements
Be able to fulfill the physical requirements of the position, including considerable walking, good hearing to assess situations quickly and respond appropriately, the ability to perform CPR and First Aid, and the ability to respond when a child is a danger to themself or others, including performing physical restraints;
Base Salary Range: $45,000.00

This position requires the ability to: identify problem/need areas; work under pressure and stressful situations in a professional manner; be thorough in assessment and documentation; locate and utilize a variety of community resources to meet the needs of each individual; assist in negotiating and advocating for each individual with service entities outside the Center; and develop a positive relationship with each consumer and their LAR where applicable. This position is responsible for direct assessment of the consumer's living environment and for providing spontaneous assistance as needs arise. This position provides a basic level of rehabilitative services addressing daily and independent living skills such as opportunities to acquire and improve skills for problem solving, accessing services and community resources, obtaining and maintaining safe, affordable housing, communicating effectively, appropriate socialization, and developing a natural support system. This position may require the use of a private vehicle for job duties/responsibilities. This position must maintain monthly performance targets. Duties are performed under the supervision of the Mental Health Program Administrator, with some latitude for the exercise of independent judgment.
Qualifications:
⢠Bachelor's degree from an accredited college or university with a major in social, behavioral, or human services (PREFER DEGREE IN SOCIAL WORK). Prefer one year of full-time work experience in the human services field, preferably including casemanagement experience.
⢠Must qualify as a QMHP-CS under the new Medicaid guidelines.
⢠Requires work or volunteer experience or expertise in one or more of the following areas: employment acquisition, housing, budgeting, community resource acquisition, etc. and the ability to communicate that knowledge to others.
⢠Must possess a current Texas driver's license and qualify for Center coverage. Driving is an essential function and employees must maintain an acceptable driving record/insurability while employed.
⢠Must have an insured personal vehicle to be used for Center duties/responsibilities.

Responsibilities
Conducts ongoing assessments to meet the needs of patients served.
Communicates between supervisors, direct care staff members, family members, guardians, and collateral individuals regarding an individual's progress.
Provides a monthly schedule to supervisors of appointments, home visits, and observations to monitor work advancement.

Qualifications
Must have experience with behavioral intervention and/or strong educational emphasis on learning and developmental psychology.
Two or more years of experience working with individuals with developmental disabilities or behaviorally challenged population.
Must have reliable transportation.
Ability to perform physical tasks, including standing for extended periods, bending, move from seated to standing regularly.
State Specific Requirements:
Illinois: Board Certified Behavior Analysts (BCBA), Licensed Clinical Psychologist, Board Certified Associate Behavior Analyst (BCaBA), or a minimum of a bachelor's degree with 1500 hours of supervised experience performed by a Licensed Psychologist or BCBA.
Indiana: A master's degree in a human related field (this includes but is not limited to BCBA, Licensed Psychologist, Licensed Counselor)
Kentucky: A master's degree in a human related field and 2 years of experience (this includes but is not limited to BCBA, Licensed Psychologist, Licensed Counselor)
Georgia: Master's degree in psychology, special education, counseling, social work, or related field OR BCBA, Licensed Professional Counselor, Licensed Clinical Social Worker, or Licensed Psychologist/Psychiatrist.
