Secures information in regard to the family unit from various sources and prepares a plan that addresses the agencies concerns and the family's goals.*
Manages payroll and supervision of one (1) person.
Facilitates team meetings and secures information which is included in weekly progress notes, outcomes, and distributes to various agencies as agreed upon at the local level within the established time frames.*
Utilizes a computer in preparing professional written reports and summaries for various agencies within the established time frame.*
When required, attends court hearings, permanency planning meetings, and other such meetings to engage and empower the families.*
Maintains records of accountability within established time frames.*
Performs administrative and/or other duties after hours, on weekends, holidays, and whenever deemed necessary by the Director of Community based Services or the Regional Vice President.*

Qualifications
License or Certification:
Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required for one's discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM or ACM™ certification upon entry into this position OR within two years of entry into the position.
CCM or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position.
· Minimum Qualifications:
For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.

Job Description
· Performs behavioral health triage and crisis management services. Links and coordinates services with medical/surgical departments and other outside agencies, such as transportation and home health care. Coordinates psychiatric diagnoses, behavioral health treatment services, and psychotropic medications with behavioral health providers and physician advisors. Key Responsibilities
Provide timely crisis assessment and intervention via phone, video, or in person

Document services accurately and maintain compliance with agency and state standards

Requirements, Skills, Knowledge and Expertise
High School/GED Required
Bachelor's or Master's in social work, psychology, or related field preferred
WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS
At least one (1) year experience working in behavioral health required if Bachelor's degree is the highest education level obtained
Working knowledge of DSM-V required
A valid driver's license and agency established minimum automobile coverage required
Reliable transportation
LICENSURE/CERTIFICATION
MO Substance Use credential required (CRADC or CADC) (if high school is highest education obtained)
PLPC, LMSW, LPC, PLMFT, LMFT, or LCSW preferred

Position Information:
Boone County Family Resources is seeking a qualified candidate for the position of Support Coordinator. Support Coordinators work collaboratively with individuals, families, and other professional team members to develop and implement individual support plans. Support Coordinators connect individuals and families with resources and services in the community and then monitor the effectiveness of those supports in helping individuals meet their needs and achieve personal goals.
Minimum Qualifications:
Applicants must be at least 21 years old, have a Bachelor's degree, and have one year of related experience working with individuals with developmental disabilities.
Additionally, applicants must have a valid driver's license, access to an insured and properly maintained vehicle for transportation, and operate that vehicle in a safe and prudent manner to comply with all applicable laws.

Schedule:
Support Coordinators work a 40-hour work week Monday through Friday. Occasional evening or weekend hours may be needed based on caseload needs. Once the introductory period is successfully completed, Support Coordinators have the opportunity to work a base schedule with some remote hours.
