We are currently looking to add a new member to our dynamic team for the position of CaseManager in our Housing Support Program. In partnership with the Department of Social Services, the Housing Support Program (HSP) provides affordable housing for homeless families or those at-risk of becoming so. Program participants are provided necessary casemanagement services for life skills development, one-on-one guidance, work group classes and community linkage to ensure their successful transition to self-sufficiency post care. The HSP CaseManager will provide one-to-one life skill development for HSP eligible participants.

Employees living outside a 50-mile radius of the San Luis Obispo Main Office (SLO) will be assigned the nearest FCNI office for in-office workdays.
Direct Service Employees are classified as Remote, as their primary worksite is considered their home. Employees are still required to attend all meetings, supervision and client meetings in-person.

COMPENSATION
Pay Range: $23.00 - $34.50 per hour
Full Time, Non-Exempt classification per FLSA
Spanish Bilingual differential

- 10% increase on hourly base rate of pay
Mileage reimbursement $0.725 cents per mile

EDUCATION & QUALIFICATIONS
Associate degree preferred, Bachelor's degree in Social Work, Psychology, or a related field preferred

Must be computer literate with proficiency in Microsoft Office Suite and knowledge of database and reporting tools

RESPONSIBILITIES
Interventions designed to improve client's social and community competencies through building or reinforcing daily living skills and budgeting that will enhance the client's ability to successfully locate and maintain housing in their community
Ability to work with clients with severe emotional needs in diverse settings.
Participate in treatment team meetings and agency specific case staffing
Prepare required written documentation, including case notes, incident reports, & agency forms
Participate in agency training, unit/staff meetings, activities, & events
Other duties and projects as assigned.

Behavioral Health Bridge Housing (BHBH) is a California-based initiative that provides temporary housing and supportive services to people experiencing homelessness who have serious behavioral health conditions. BHBH CaseManagers are expected to supervise the general health, safety, and social rehabilitation of clients. Responsibilities and Duties:
1. Resident Services: Includes participating in intake interviews, facilitating house meetings, meeting with residents weekly, and on an as needed basis for casemanagement, advocacy, and ILS counseling purposes, providing information and referral services for community resources.
2. Provide crisis intervention, and communicate the emotional, physical, and mental state and functioning of clients to the Program Manager and other staff members by attending scheduled staff meetings and by completing daily, weekly and monthly charting.
3. Site management: Includes inspecting sites, assisting with cleaning and/or repairs, arranging for professional repairs or services, participating in the purchase or donation of furniture or household supplies, communicating with the Program Manager regarding physical repairs, enhancements, and/or cleaning that is needed. Assist residents with move in/out activities.
4. Liaison: Communicate with other service providers regarding current clients, communicate with the Housing Authority, when appropriate, regarding client intake, rents, and Section 8 status, attend direct service provider meetings, and serve as a representative of the program to the community.
5. Extended Services: Provide casemanagement and support services to clients who have moved on to independent housing situations. When applicable, serve as a Section 8 resource and aid the client throughout the process, work closely with the Housing Navigator to assist the client in all aspects of attaining independent housing, and communicate the progress of extended service clients to the Program Manager and other staff members through staff meetings and charting.
6. Complete program paperwork. This may include, but is not limited to individual client files, evaluating and charting client progress, service plans, admission/ discharge paperwork, HMIS data entry, Excel, Word, One Note, collect and record client rents, enter data into electronic record keeping system for MediCal billing, and security deposit receipts, and other required forms. Ability to meet program deadlines.
7. Medi-Cal billing provides a critical portion of funding for this program. You are expected to reach documentation and billing goals as set by Team Leader and Director.
8. All direct service staff are expected to spend at least 50% of their work time in direct service and support of clients, unless otherwise assigned by your supervisor.
9. Promote, encourage and maintain trauma-informed practices in all services and activities. Practice and promote, both individually and in team meetings, TMHA's Collaborative Communication Guidelines in order to facilitate clear, empathetic and person-centered communication.
10. Attend weekly staff meetings.
11. Assume other duties and responsibilities as assigned by Program Manager and Program Director.

The Certified/Registered SUD Counselor working under limited supervision is responsible for providing services to substance abuse clients; to include, assessments, developing treatment and recovery plans, identifying behaviors that impede rehabilitation, and facilitating individual and group counseling sessions.
How will I contribute
Deliver individual and group counseling services, casemanagement services, education and community outreach services, and referrals to appropriate services/community resources as necessary.
May provide intake and program orientation for participant including, but not limited to, assessment of psychological and social functioning and the development, implementation, and ongoing review of the participants Individual Treatment Plans in accordance with the Title IX regulations and Program protocol and policies.
Conduct breathalyzer and/or saliva tests on participants as appropriate, documenting results
Prepare participant progress reports, when necessary.
Interact with the criminal justice system and/or referral sources, when necessary.
Monitor and maintain participant files, charts, attendance schedules, face-to-face monitoring forms, group progress notes, completion of vocational/educational and self-help activities, aftercare, scheduled make-up sessions, etc. to ensure compliance with Title IX regulations and Program policies.
Monitor participant payments/fees and assist with fee collections. Conduct financial assessment interview in accordance with Title IX Regulations and Program protocols.
Recommend leaves of absence, dismissal/suspension, reinstatement, completion, etc. for participants in accordance with Title IX and Program policies.
Communicate openly and clearly, on a regular basis, with Clinical Supervisor and support staff to ensure Program effectiveness (appropriate caseload, work hours, scheduling, group capacity, etc.)
Abide by all Kings View policies and procedures, including but not limited to, confidentiality of program participants (Title IX, Title 42, and HIPPA/Privacy).
Attend scheduled staff meetings.

Associate degree or the equivalent sixty college semester units with emphasis in behavioral or social sciences if required for the position.

Key Responsibilities:
Provides orientation to new patients to the programs' rules, policies, procedures, and patients' rights.
Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans professionally and within time guidelines as required by applicable federal and state regulation, CARF guidelines and Pinnacle Policies.
Involve patients in the assessment, treatment planning, and intervention aspects of treatment, which provides the patient with a sense of entitlement to their own recovery.
Administer the biopsychosocial assessment, which evaluates all areas of patients' lives, including their addictions, medical and mental health history, socioeconomics, legal, etc. at intake and on an ongoing basis.
Recognize and utilize patients' Strengths, Needs, Ability, and Preferences (SNAP), to develop and implement individualized Treatment Plans that effectively help patients to meet their short & long-term goals. • Determine the clinical necessity of counseling services based on the clinical assessment and evaluation of the patients.
Monitor closely, and document any change of circumstances with the patients, especially as it relates to matters that may affect their recovery, identifying and addressing relapse indicators to promote relapse prevention interventions in an early stage.
Comply with and implement the Medical Q.A (Quality Assurance) and Clinical Risk Management Policies, including:
Evaluate the patients for any high-risk conditions (e.g., liver failure, pregnancy, overdose, bipolar, schizophrenia, etc.), which may endanger the patient (suicidal) or other parties (homicidal).
Administer the BAM-R, C-SSRS, GAD-7, and PHQ-9 screening tools to determine the severity of the patient's mental/emotional status.
Evaluate the risk factors concerning each such case, considering the appropriateness of, and professionalism in treating such individuals in an OTP in comparison to treatment in a residential facility or other treatment modalities.
Review and ask about other practitioners' involvement in the treatment of these high-risk conditions. Recommend immediate referral to such a specialist in case of need. Request the patient sign a waiver for release of information which will allow communication with the other practitioner in case of need (e.g., deterioration of the patient's condition).
Flag high-risk patients to the Medical Directors, Physician Extenders, and Regional Clinical Leadership (RCL) and highlight the conditions concerned.
Refer such patients to the Medical Directors, Physician Extenders or RCLs (Regional Clinical Leadership) based on the urgency, utilizing the informal Referral Policy or the scheduled Treatment Team/Case Conference session.
Review physician notes, following the Medical Directors or Physician Extenders' session with the patient, verifying that they noted and addressed the high-risk factor, or otherwise bring it immediately to the attention of the supervising counselor, as well as the clinicmanager, until the matter's proper and professional handling by the physician or physician extender.
Advise, in writing, other staff members (including the clinicmanager, supervising counselor, dispensing nurse and front office) of high-risk patients and their condition. Maintain and circulate minutes from treatment team/case conference sessions to the other staff members.
Provide professional counseling and referral services by:
Addressing patients' clinical and special needs by maintaining referrals to specialized (“second tier”) services rendered by physicians, physician extenders and counselors within Aegis, as well as referrals to Keys to Recovery (or other twelve-step) support groups, and other community resources (e.g., agencies and practitioners outside of Aegis), acting as a liaison to these agencies, as necessary.
Providing individual and group counseling as clinically necessary, to address the problem issues identified in the Initial Assessment and Treatment Plans. The counseling service will include protocols for the treatment of Relapse Prevention, Peer Pressure, Anger Management, Domestic Violence, Parenting, Family Preservation, Vocational Rehabilitation, etc.
Utilizing advanced counseling techniques such as Cognitive Behavioral Therapy, Motivational Interviewing, etc.
Scheduling regular counseling sessions with patients; issue patient appointment cards and record scheduled or rescheduled sessions in EHR scheduler.
Documenting counseling entries, including signature and date, as required by applicable federal and state regulations, CARF guidelines and Pinnacle policies.
Engaging in Community Relations and Outreach efforts.
Attending all recommended training including Physician Training of Counselors, RCL training, and case conferences with the program physicians and physician assistants.
Collecting research data when requested.
Observing the collection of patients' urine samples when required
Attend team meetings and complete all training courses timely as required.

1. Resident Services: Includes participating in intake interviews, facilitating house meetings, meeting with residents weekly, and on an as needed basis for casemanagement, advocacy, and ILS counseling purposes, providing information and referral services for community resources.
2. Provide crisis intervention, and communicate the emotional, physical, and mental state and functioning of clients to the Program Manager and other staff members by attending scheduled staff meetings and by completing daily, weekly and monthly charting.

Ability to meet program deadlines.
7. Medi-Cal billing provides a critical portion of funding for this program. You are expected to reach documentation and billing goals as set by Team Leader and Director.

Title: Service Coordinator (Mental Health CaseManager)
Job Duties: Under the supervision of the Program Director, the Service Coordinator (Mental Health CaseManager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health CaseManager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health CaseManager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.
Schedule: Part-Time
Qualifications:
Two (2) years of full-time experience or part-time equivalent in a mental health setting.
A bachelor's degree with a major in psychology, social work or behavioral sciences may be substituted for the two (2) years of full-time work experience or its part-time equivalent.
Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
Familiarity with regulatory standards.

The salary range listed below represents the minimum and maximum base pay per hour at the time of posting. Final salary offered to the candidate selected for the position will be based on factors including but not limited to candidate's skills, experience, licensure, and program acuity. Pay Range:$23-$25 USD

Southern San Luis Obispo areas (5 cities- Lompoc).
This role offers the opportunity to make a meaningful impact while enjoying autonomy and flexibility, supported by clinical supervision from an experience LCSW. Clinical supervision is available for candidate working toward licensure.
Requirements:
Bachelor's Degree in Sociology, Psychology, Human Development OR
Master's Degree from a school accredited by the Council on Social Work Education
Minimum of one-year social work experience in a healthcare setting, home health &/or hospice experience preferred.
Understand the "hospice philosophy" and issues of death and dying
Reliable vehicle to drive for work
