Responsibilities
Summary
Provides case management of Intensive Supervision Appearance Program (ISAP) participants from entry to release by administering the program's policies and procedures as defined by the Department of Homeland Security (DHS) contract.
Primary Duties and Responsibilities
Develops, implements, coordinates, reviews and updates case management and individual service plans for ISAP participants in conformance with state and federal regulations and program requirements. Meets with participants face-to-face regularly as required by contractual obligations.
Develops and maintains accurate and complete case records for all ISAP participants from entry to release from ISAP. Proactively manages cases using all appropriate tools and techniques. Maintains confidentiality of all records.
Conducts intake interview and orientation session with each ISAP participant within contractual timeframes and requirements.
Installs electronic monitoring equipment on ISAP participants as necessary and monitors compliance with electronic monitoring program.
Assists ISAP participants with acquiring travel documents from the countries of citizenship. Maintains a log of travel document information for each participant.
Evaluates English proficiency of ISAP participants to determine resources necessary to promote clear communication between ISAP staff and participants.
Coordinates and facilitates translation services for ISAP participants, including emergency translation services.
Connects participants with community resources including, but not limited to, transportation, medical, food, shelter, clothing, educational, financial, legal, religious and other social adjustment or facilitative resources. Identifies and coordinates services as required.
Supervises ISAP participants while in Company offices.
Establishes schedules for ISAP participants that include, but are not limited to, counseling sessions, immigration Court appearances, and access to legal services.
Coordinates with participants to ensure attendance at legal orientation and cultural orientation sessions, as well as supplemental services evaluations.
Reviews individual service plans, conducts risk/needs assessments and writes progress reports on a regular basis in compliance with contractual obligations.
Reports on program violations in a timely manner using established reporting parameters.
Conducts home visits to verify participants' places of residence in compliance with contractual obligations.
Performs emergency counseling for ISAP participants as needed.
Identifies and maintains relationships with translation resources in the community, including private services, volunteers, universities, English as a Second Language (ESL) schools, NGOs, churches, and other organizations that might have resources to offer.

Qualifications
Minimum Requirements
Bachelor's Degree required. Degree in Sociology, Psychology, Social Work, Criminal Justice or related field preferred.
At least two (2) years of customer service or case management experience required; experience in fields related to law, social work, detention, corrections or working with multi-cultural clients preferred.
Bilingual English/Spanish required.
Valid driver's license required and the ability to safely operate a motor vehicle in order to perform home visits.

The FSP Case Manager will work as a member of the Homeless Outreach Team (FSP Team) to manage a caseload of adults with serious mental illness who are unhoused or at risk of becoming unhoused. 1. Provide case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team; assume primary responsibility for developing, writing, implementing, evaluating and revising treatment goals and plans; adhering to all required reporting requirements; provide one-on-one support to clients; provide symptom management support; educate and support clients' families while acting in the clients' best interest and advocating for the rights of the clients. 3. Provide ongoing assessment of clients' psychiatric symptoms and their responses to treatment. Recognize, be available and provide one-on-one support for clients in psychiatric crisis or emotional distress, and assist clients in developing appropriate coping and conflict resolution skills. Provide counseling, emotional support, and symptom recognition and management. Educate clients and their families about the nature of mental illness, symptoms of mental illness, coping strategies, and medication. Assist clients in developing ways to manage symptoms, augment coping skills, and strategize problem-solving skills.
4. Assist clients in learning more about substance abuse and treatment options open to them such as A.A. and other support groups. Work with clients to identify possible precursors to alcohol or drug abuse.
5. Assume the role of an advocate in all aspects of clients' needs and goals. This duty may include familiarizing clients with and encouraging the usage of community resources, support systems, housing opportunities, medical services, and opportunities; and assisting clients in building healthy peer and familial support networks. Assist clients in developing various recreational skills.
6. Discuss with clients what goals they plan to achieve for them-selves and assist clients in determining, developing, and carrying out their own goals; review plans and goals, evaluate progress, and strengthen independence.
7. Provide ongoing assessment of activities of daily living. Assist clients in developing skills in the following areas, as needed, in order to achieve the highest level of independence possible: personal hygiene, shopping and cooking skills, money management skills, accessing reliable transportation, and scheduling / attending appointments.
8. Provide opportunities to allow clients to establish vocational goals and identify any job-related areas that they want to work on. Provide vocational counseling, referral to appropriate vocational services, and assist clients with job coaching and support as needed.
9. Supervise the cleanliness and upkeep of the residences; this duty may include general household chores, yard work, completion and/or reporting of needed repairs, acquisition and disposal of furniture, and teaching and assisting clients in maintaining a clean and sanitary living environment.
10. Complete program paperwork such as individual client files, evaluating and charting client progress, treatment plans, admission/discharge paperwork, Medi-Cal billing, inter- and intra-agency service referrals, petty cash receipts, vehicle, and mileage tracking, etc.
11. Promote, encourage and maintain trauma-informed practices in all services and activities. Practice and promote, both individually and in team meetings, TMHA's Collaborative Communication Guidelines in order to facilitate clear, empathetic and person-centered communication.
12. Attend regular staff meetings, participate in client intake meetings, and complete required hours of in-service training.

Job Summary:
The RN Case Manager is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility for ensuring that care is provided at the appropriate level of care based on medical necessity and to assess the patient for transition needs to promote timely throughput, safe discharge and prevent avoidable readmissions. This position integrates national standards for case management scope of services including: • Utilization Management supporting medical necessity and denial prevention • Transition Management promoting appropriate length of stay, readmission prevention and patient satisfaction • Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care • Compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy • Education provided to physicians, patients, families and caregivers The individual's responsibilities include the following activities: a) accurate medical necessity screening and submission for Physician Advisor review, b) care coordination, c) transition planning assessment and reassessment, d) implementation or oversight of implementation of the transition plan, e) leading and facilitating multi-disciplinary patient care conferences, f) managing concurrent disputes, g) making appropriate referrals to other departments, h ) identifying and referring complex patients to Social Work Services, i) communicating with patients and families about the plan of care, j) collaborating with physicians, office staff and ancillary departments, k) leading and facilitating Complex Case Review, l) assuring patient education is completed to support post-acute needs , m) timely complete and concise documentation in Case Management system, n ) maintenance of accurate patient demographic and insurance information, o) identification and documentation of potentially avoidable days, p) identification and reporting over and underutilization, q) and other duties as assigned. Essential Functions:
Assists physicians in making admission decisions considering the use of observation, outpatient and inpatient status and payor guidelines.
Demonstrates clear and effective communication with nursing and physicians. Works cooperatively with nursing, dietary, P.T. and social services to develop a safe discharge plan. Maintains patient, co-worker, and hospital privacy.
Documentation is clear, concise, and legible. Develops a discharge plan that is safe and meets the patient's needs. Need for follow-up case management is based on age, physical, psycho-social, financial or any other identified need which may affect progress towards return home or the next appropriate level of care. Obtains psycho/social assessment according to discharge planning protocol for all age groups. Reviews medical history, physical examination, and all available assessment data to assist in developing the discharge plan. Collects information needed to begin discharge planning in a timely manner, and initiates contact with patient and/or family. Reviews care and progress on assigned patients as often as necessary, but no less than every 72 hours and documents in the discharge planning notes. Coordinates team or family conferences as needed on complex patients. Ensures that the patient is medically stable for discharge from the inpatient setting and that the patient and family are prepared educationally, emotionally and functionally for discharge. Maintains a reference network of health systems and community resources in order to adequately plan for discharge. Uses Interqual and CMS programs for assessment and discharge planning.
Assist in the development and education of new staff members. Attends and participates in staff meetings. Completes mandatory modules as required. Successfully adapts to changes in the unit, environment, and workflow.
Obtains optimal coverage for patients by furnishing appropriate medical information to insurance carriers. Notifies review companies of planned treatment, length of stay, progress, etc. and obtains initial certification. Refers appropriate patients to the MEP (financial counselor). Presents letters of denial of coverage when appropriate.

Key Responsibilities:
Provides orientation to new patients to the programs' rules, policies, procedures, and patients' rights.
Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans professionally and within time guidelines as required by applicable federal and state regulation, CARF guidelines and Pinnacle Policies.
Involve patients in the assessment, treatment planning, and intervention aspects of treatment, which provides the patient with a sense of entitlement to their own recovery.
Administer the biopsychosocial assessment, which evaluates all areas of patients' lives, including their addictions, medical and mental health history, socioeconomics, legal, etc. at intake and on an ongoing basis.
Recognize and utilize patients' Strengths, Needs, Ability, and Preferences (SNAP), to develop and implement individualized Treatment Plans that effectively help patients to meet their short & long-term goals. • Determine the clinical necessity of counseling services based on the clinical assessment and evaluation of the patients.
Monitor closely, and document any change of circumstances with the patients, especially as it relates to matters that may affect their recovery, identifying and addressing relapse indicators to promote relapse prevention interventions in an early stage.
Comply with and implement the Medical Q.A (Quality Assurance) and Clinical Risk Management Policies, including:
Evaluate the patients for any high-risk conditions (e.g., liver failure, pregnancy, overdose, bipolar, schizophrenia, etc.), which may endanger the patient (suicidal) or other parties (homicidal).
Administer the BAM-R, C-SSRS, GAD-7, and PHQ-9 screening tools to determine the severity of the patient's mental/emotional status.
Evaluate the risk factors concerning each such case, considering the appropriateness of, and professionalism in treating such individuals in an OTP in comparison to treatment in a residential facility or other treatment modalities.
Review and ask about other practitioners' involvement in the treatment of these high-risk conditions. Recommend immediate referral to such a specialist in case of need. Request the patient sign a waiver for release of information which will allow communication with the other practitioner in case of need (e.g., deterioration of the patient's condition).
Flag high-risk patients to the Medical Directors, Physician Extenders, and Regional Clinical Leadership (RCL) and highlight the conditions concerned.
Refer such patients to the Medical Directors, Physician Extenders or RCLs (Regional Clinical Leadership) based on the urgency, utilizing the informal Referral Policy or the scheduled Treatment Team/Case Conference session.
Review physician notes, following the Medical Directors or Physician Extenders' session with the patient, verifying that they noted and addressed the high-risk factor, or otherwise bring it immediately to the attention of the supervising counselor, as well as the clinic manager, until the matter's proper and professional handling by the physician or physician extender.
Advise, in writing, other staff members (including the clinic manager, supervising counselor, dispensing nurse and front office) of high-risk patients and their condition. Maintain and circulate minutes from treatment team/case conference sessions to the other staff members.
Provide professional counseling and referral services by:
Addressing patients' clinical and special needs by maintaining referrals to specialized (“second tier”) services rendered by physicians, physician extenders and counselors within Aegis, as well as referrals to Keys to Recovery (or other twelve-step) support groups, and other community resources (e.g., agencies and practitioners outside of Aegis), acting as a liaison to these agencies, as necessary.
Providing individual and group counseling as clinically necessary, to address the problem issues identified in the Initial Assessment and Treatment Plans. The counseling service will include protocols for the treatment of Relapse Prevention, Peer Pressure, Anger Management, Domestic Violence, Parenting, Family Preservation, Vocational Rehabilitation, etc.
Utilizing advanced counseling techniques such as Cognitive Behavioral Therapy, Motivational Interviewing, etc.
Scheduling regular counseling sessions with patients; issue patient appointment cards and record scheduled or rescheduled sessions in EHR scheduler.
Documenting counseling entries, including signature and date, as required by applicable federal and state regulations, CARF guidelines and Pinnacle policies.
Engaging in Community Relations and Outreach efforts.
Attending all recommended training including Physician Training of Counselors, RCL training, and case conferences with the program physicians and physician assistants.
Collecting research data when requested.
Observing the collection of patients' urine samples when required
Attend team meetings and complete all training courses timely as required.

Under the direction of the Assistant Regional Director, the Outpatient Clinician is responsible for providing and implementing the clinical therapy component of the overall treatment plan. DUTIES AND RESPONSIBILITIES (illustrated by typical activities):
Provides clinical services such as individual, family, and group therapy, and parent skills training to clients ages 5 - 24.
Utilizes evidence-based practices that are appropriate to meet the clinical needs of the individual and family.
In collaboration with the clinical supervisor, Conducts clinical assessments and develops individualized treatment plans, including Strength and Needs assessments.
Collaborates with other community providers and make appropriate referrals as needed, to provide comprehensive care for the individual.
Responsible for maintaining organized and up-to-date clinical charts in compliance with insurance standards and agency policies.
Completes clinical documentation and submits progress notes to the clinical supervisor in agency electronic health records within the required time frame.
Attends outside training events and conferences as requested.
Maintains compliance with all local, County, State, and Federal government regulations.
Attends weekly individual and group clinical supervision.
Perform other job duties as assigned.
EDUCATION and/or EXPERIENCE/LICENSE: Hold at minimum valid registration ACSW, AMFT, APCC or Registered Psychological Assistant.
OTHER SKILLS AND ABILITIES:
Ability to use various computers Software, Microsoft Office Suites.
Bilingual in Spanish required.
Strong organizational, communication and interpersonal skills.
Valid driver's license is required for transporting youths. Any changes that occurs on your driving record must be reported immediately to supervisor and Human Resources.
Language Skills: Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. Ability to write comprehensive evaluations and progress notes/ reports on youth's treatment goals. Bilingual in Spanish a plus
Mathematical Skills: Ability to add, subtract, multiply and divide in all units of measure.
