To provide leadership to the casemanagement team; to support, guide, and mentor Hope Street guests throughout the program, providing services to guests, coordinating services, assisting guests in progressing toward their career goals and in connecting with possible employers and follow up with participants when they leave the program. This position will report to the Hope Street of Siouxland Executive Director.
Essential Job Functions
Plan with the Director for optimal program success by beginning of guest enrollment
Provide intensive, face-to-face casemanagement to enrolled guests; meet with guests at least weekly at enrollment, or more as needed; continue to communicate with guests as they transition out of the house
Assess guests to determine individual and family needs, physical and mental health needs, independent living skills, academic needs, and employability
Assist guests in setting realistic and achievable goals
Screen guests admitted to the program for indicators of mental health and substance abuse issues
Refer guests to partners and community providers for needs not provided by Hope Street
Contribute to the development of outreach materials, including social media, to recruit guests and employer partners and donors
Assist guests in selecting vocational training, meeting training admission requirements and facilitate the enrollment into the training
Assist guests in their need for transportation, driving guests to appointments as needed
Guide, oversee and monitor service learning and community involvement
Prepare guests for daily living skills and employment
Establish and coordinate accurate records of all casemanagement activity
Develop and maintain a positive working relationship with program partners
Ensure that casemanagement includes culturally responsive elements to ensure services are inclusive to all
Help create an environment that is safe, drug and alcohol free, and conducive to teaching and learning; report any observed guest problem behaviors to the Director
Contribute as a positive and productive role model for the guests but also other program staff and volunteers

Education Requirements
BA or BS Degree in Social Work, Education or closely related discipline preferred, or the equivalency in years of casemanagement
Experience/Qualification Requirements
At least 3 years of casemanagement experience
Experience working with unhoused and/or incarcerated individuals preferred
Good written and verbal skills
Public speaking skills
Self-motivated to take responsibility for the success of guests and the program
Demonstrated ability to organize and prioritize work responsibilities with limited supervision and/or structured guidance

Requirements:
Bachelor's degree or an associate of arts degree in human services or related field plus two years of full-time experience in human services or related field.
Job Responsibilities:
Provide in-home intervention to children and their families utilizing skill-based, evidence-based practices to help families implement solutions to problems and strengthen families.
Act as a resource, provider & advocate families to promote child welfare.
Provide evidence-based interventions and casemanagement support under contractual responsibilities.
Utilize collaborative documentation.
Develop and maintain effective working relationships with community partners including DHHS, medical providers, “parent partners,” lawyers, therapists, law enforcement staff, school officials, neighbors, fictive kin, emotional supports, and referral sources.
Provides evidence-based practices to families and individuals associated with the family, utilizing Solution-Based Casework, SafeCare, Child Safety Conferences, Motivational Interviewing, SFM & YTDM meetings.
Maintain an appropriate level of direct contact with families per program requirements and remain available to families 24/7 for crisis intervention as needed.
Ensure professionalism through role modeling, and respect.
Utilize own vehicle to travel to work destinations and transport youth as authorized, provide services in the community, or in the primary home of the child and family.
Report and document emergencies and critical incidents involving family members or staff.
Participate actively in individual, group, and organizational supervisions/trainings.

The Ideal Candidate Will Have:
A bachelor's degree in social work, psychology, sociology, public administration, or other human service-related fields is preferred. Entry-level knowledge and abilities may be acquired through any combination of training and/or experience that will enable the incumbent to possess the required knowledge, skills, and abilities
Knowledge of:
* theories and concepts related to culture, child development, family connections and family dynamics and their influence on child and parental relationships and behavior;
