We're looking for someone to join our team as a HIM/Medical Records Coordinator who:
Under general supervision and in accordance with established policies and procedures, analyzes and maintains electronic medical charts filing system with accuracy, consistency, and completeness. Reviews and processes request for subpoenas received from outside attorneys, agencies, providers, and schools in compliance with applicable state laws. Performs a variety of clerical duties related to processing of electronic medical records and requests. Assist in the preparation of charts for patient's visits, audits, and to file their patients' reports.
Essential Functions:
* Upholds Clinica Sierra Vista's Policies and Procedures, HIPAA, Compliance, Principles of responsibilities, and applicable state, federal, and local laws.
* Generate barcodes in EPIC EMR system module to identify patient data and demographic for electronic filing and analyzes electronic charts for accuracy, consistency and completeness.
* Usage of OnBase scanning module in order to scan batches of various medical records documents to upload documents through batch to index interphase into OnBase Indexing processing module.
* Usage of OnBase Production module for processing with incorporated data fields with attention to detail in the description to index, commit, and batch various types of patient medical records documents and/or reports through OnBase Production mode module to interphase in EPIC EMR files for end-user accessibility of records for continuity of care and services.
* Through automatization workflows, maintains EPIC ROI electronic Module for various types of Release of Information requests and identifying the requestor as Third Party, Patient, relation, and/or Provider request by verifying demographics. Fills in data with hard stops to complete module.
* Determines Medical Records Billing flow as "Do Not Bill, Pre-Pay, and/or Post-Pay. Enters all aspects and information of the billing and release address within the ROI module.
* Identifies the Release type and purpose of the request within the ROI module. Scans the authorization type documents and/or request by identifying the authorization type, the description, and expiration within the ROI module.
* Filters and identifies the request date range and type of information requested and produces a query within the ROI module. Keep track of comments, dates of requests, Date Need by, priority, and assignment of HIM/Medical Records Clerk in the data fields within the EPIC ROI Production Module system.
* Filters and generates outputs of EMR reports in order to fulfill to fulfill the requests based on the requestor's instructions. Completes the status of the Release through EPIC ROI module for tracking purposes.
* Maintains assigned ROI Releases for tracking purposes of all requests, ensuring the ROI functions for HIPAA is completed.
* Generates and electronically save Medical Records Invoices through incorporated EPIC Letters Modules ensuring the correct information and patient is extracted from the patient's demographic electronic medical record file.
You'll be successful with the following qualifications:
* High school Diploma required.
* EMR experience. EPIC experience preferable.
* Typing a minimum 35 WPM and proficient computer skills; including but not limited to Microsoft products and use of outlook

Prioritizes release of information requests. STAT requests are prioritized based on the need for Continuum of Care, order received, otherwise processed no later than end of business day. Handles all requests and inquiries for patient health information whether received via mail, HXE, fax, phone or in person. Verifies identity and confirms that the authorization is valid. Ensures the requesting party has a legal right to request a patient's medical information. Responds to and fulfills requests from patients, third party payors, auditors, providers, compliance, etc. Ensures all release requests are HIPAA compliant and verifies all releases for accuracy. Reviews inpatient and outpatient medical records for completion, accuracy, and compliance with regulations. Enters chart deficiencies into the computerized system for each incomplete or missing document. Updates deficiencies and completed documentation as necessary no later than five (5) days from patient discharge. Monitors and clears on hold tasks and the reanalysis queue daily. Maintains and monitors report of charts pending physician completion. Notifies responsible physicians and medical staff of record delinquency. Serves as a resource and liaison for physician issues or questions in the completion of records, dictated reports or reports created in the electronic health record so as to affect a smooth work flow. Regularly communicates with physicians and other health care professionals to obtain incomplete documentation. Maintains knowledge related to documentation requirements necessary to perform job. Reviews the Refused Verbal Order report on a weekly basis and re-assigns orders to the appropriate party.
DATA INTEGRITY ALSO RESPONSIBLE FOR:
Identifies and corrects duplicate medical record and guarantor numbers. Utilizes reports from the Soarian Financials System which identifies numbers that have been assigned in error. Identifies and corrects duplicate guarantors within the medical record. Transfers scanned documents in medical records that have been changed from outpatient to inpatients or other status changes to ensure only one complete record. Reviews request to remove erroneous documentation and delete if found to be a valid request. Creates Home Health accounts with information provided from admit list report. Reviews loose Home Health Documents in attempts to identify corresponding encounter, episode number and/or create account/MRN if one not found.
PATIENT PORTAL ALSO RESPONSIBLE FOR:
Serves as the subject matter expert on patient portal functionality. Assists patients who are not able to establish portal access through self-registration by sending them an invitation to MyHealth Portal. Interacts with patients and patient representatives to establish proxy access. Assures that appropriate consent is obtained prior to establishing proxy access. Serves as the primary Kaweah Health contact for MyHealth Portal. Answers calls in prompt and professional manner. Responds to email and voicemail requests in a timely and efficient manner.
VITAL STATISTICS ALSO RESPONSIBLE FOR:
Serves as a liaison and main point of contact for funeral homes and physicians to coordinate the completion of death certificates. Runs a daily census and identifies patients who have expired in the medical center in order to begin the death certificate process. If contacted by a funeral home, initiates the death certificate process in not already identified via the census. Identifies the physician responsible for providing causes of death and completion of death certificate. Issues attestations to physicians in a timely manner. We're looking for someone to join our team as a HIM/Medical Records Coordinator who:
Under general supervision and in accordance with established policies and procedures, analyzes and maintains electronic medical charts filing system with accuracy, consistency, and completeness. Reviews and processes request for subpoenas received from outside attorneys, agencies, providers, and schools in compliance with applicable state laws. Performs a variety of clerical duties related to processing of electronic medical records and requests. Assist in the preparation of charts for patient's visits, audits, and to file their patients' reports.
Essential Functions:
Upholds Clinica Sierra Vista's Policies and Procedures, HIPAA, Compliance, Principles of responsibilities, and applicable state, federal, and local laws.
Generate barcodes in EPIC EMR system module to identify patient data and demographic for electronic filing and analyzes electronic charts for accuracy, consistency and completeness.
Usage of OnBase scanning module in order to scan batches of various medical records documents to upload documents through batch to index interphase into OnBase Indexing processing module.
Usage of OnBase Production module for processing with incorporated data fields with attention to detail in the description to index, commit, and batch various types of patient medical records documents and/or reports through OnBase Production mode module to interphase in EPIC EMR files for end-user accessibility of records for continuity of care and services.
Through automatization workflows, maintains EPIC ROI electronic Module for various types of Release of Information requests and identifying the requestor as Third Party, Patient, relation, and/or Provider request by verifying demographics. Fills in data with hard stops to complete module.
Determines Medical Records Billing flow as “Do Not Bill, Pre-Pay, and/or Post-Pay. Enters all aspects and information of the billing and release address within the ROI module.
Identifies the Release type and purpose of the request within the ROI module. Scans the authorization type documents and/or request by identifying the authorization type, the description, and expiration within the ROI module.
Filters and identifies the request date range and type of information requested and produces a query within the ROI module. Keep track of comments, dates of requests, Date Need by, priority, and assignment of HIM/Medical Records Clerk in the data fields within the EPIC ROI Production Module system.
Filters and generates outputs of EMR reports in order to fulfill to fulfill the requests based on the requestor's instructions. Completes the status of the Release through EPIC ROI module for tracking purposes.
Maintains assigned ROI Releases for tracking purposes of all requests, ensuring the ROI functions for HIPAA is completed.
Generates and electronically save Medical Records Invoices through incorporated EPIC Letters Modules ensuring the correct information and patient is extracted from the patient's demographic electronic medical record file.
You'll be successful with the following qualifications:
High school Diploma required.
EMR experience. EPIC experience preferable.
Typing a minimum 35 WPM and proficient computer skills; including but not limited to Microsoft products and use of outlook
