POSITION SUMMARY:
The Social Services Director is responsible for supervision and staff development, program planning and development, and client services.

Rapidly after release into the program, the Clinical Case Coordinator conducts Supervision Introduction/Orientation meetings with new participants to review supervision conditions that will be required under the supervision plan. The Clinical Case Coordinator assists participants to comply with supervision appointments and court appearances during the pendency of their legal case. In this role, the Clinical Case Coordinator will conduct ongoing service need assessments with participants and develop collaborative service plans addressing identified needs and goals. The Clinical Case Coordinator will ensure participants receive services that are strengths-based, gender-specific, and culturally sensitive while adhering to the core principles of risk, need and responsivity to reduce recidivism.

What you will be doing:
Provide supervision and community-based services to a caseload of pretrial participants.

Ensure the timely flow of community outreach and case management activities to meet program goals and objectives.
Utilize problem solving techniques, motivational interviewing as an integral part of the pretrial supervision and case management process approach, to help motivate and engage individuals in any needed process of change.
Provide all services in a manner that is gender-responsive, trauma-informed, and that adheres to the core principles of risk-need-responsivity.
Maintain collaborative relationships with community providers (supported housing providers, Health Homes, care management programs, addiction treatment and mental health providers, peer-run providers, education, vocational. training and employment services providers, legal services providers, medical care providers, and family providers).
Follow program community-based monitoring protocols for high risk and high need program participants and request/participate in case reviews to address complex cases.

Meet monthly case management contact and outreach goals as established by supervisory staff.

The QUNO New York office is legally and administratively part of the AFSC, based in Philadelphia. At present, the QUNO New York office is using a hybrid model of work with staff working both in-person and from home.
ESSENTIAL FUNCTIONS / RESPONSIBILITIES AND ACCOUNTABILITIES
Participate in the administrative functioning of the office and Quaker House, including phone answering; processing mail and responding to email; supporting the maintenance of Quaker House; greeting visitors; managing planning, logistics, and preparations required to host program activities; and taking notes and maintaining records in support of QUNO program activities.
Attend and participate in all QUNO staff-related meetings where appropriate.
Specialize in several areas of work under the QUNO strategic plan and help provide coverage of the work under the supervision of senior staff. The duties and responsibilities of the Residence Program Specialist will include but are not limited to the following:
Provides care and assists with personal development and independence of the people we support.
Ensures that all measures for safety and well-being are maintained at highest level.
Provides training for the people we support in activities of daily living and in development of self-help and social skills.
Maintains a personalized, homelike atmosphere in the residential facility.
Attends weekly staff meetings as well as any other assigned meetings and in-service trainings.
Prepares concise written reports including Incident Reports, Progress Notes and other reports as assigned.
Responds to crisis situations and medical emergencies, including accidents by taking quick and decisive action.
Ensures actions are conducted in a manner that prevents abuse, and reports observations of co-workers who may be abusive in accordance with established procedures.
Administers appropriate first aid based on visible symptoms and medical history of the people we support in accordance with established guidelines and accepted first aid practices.
Summons police, fire, ambulance or other emergency personnel if situation warrants it.
If an emergency has caused a person we support to exhibit behavior that is threatening or dangerous, may have to utilize strategies for crisis intervention in accordance with OPWDD approved Behavior Management guidelines.

Shortly afterward, you knocked on doors to check on seniors who missed a recent wellness visit before working with the local health center to plan a health fair. Your stack of reports and applications still needs to get done, but at heart everyone knows you are the caring person they can turn to. You are the new Community Coordinator at LSA Family Health Service's first housing-based office, bringing health, hope, and connection to the community.

Salary: The salary for the Clinical Case Coordinator role is $69,010 per year.
Shift Hours: The Clinical Case Coordinator is working Monday through Friday from 9:00 am to 5:00 pm.
Location Address: 151 Lawrence Street, 4th Floor, Brooklyn, NY 11201.
Workplace Flexibility: On-site (for roles that are 100% in-office/in-person)
What you will be doing:
Will provide community-based direct services to a limited, specialized caseload of pretrial participants that may include a higher recidivism risk, participants with mandated treatment requirements, and those individuals with higher profiled cases.
Rapidly at the point of arraignment conduct Intake Assessments to identify immediate critical needs that must be addressed to further sustain program involvement and enrollment.
Participate in regularly occurring team meetings and case conferences to ensure seamless coordination among staff, wide dissemination of participant information as well as rapid responses to urgent needs.
Facilitate effective collaboration with staff delivering court services to ensure the efficient enrollment of new participants into the program while gathering the necessary basic information for ongoing service provision.
Link participants to emergency, housing, education, vocational, employment, and treatment services based on their preferences and needs, provide navigation to community services, and establish effective collaboration and positive relationships with community providers.
Will conduct review of legal and court screening documentation to facilitate admission and ongoing needs assessment of criminal history, psychosocial, developmental, and behavioral health needs, supervision risks to identify engagement approach, focus for behavioral change, and needed supports to reduce re-arrest, and promote success in the community following program completion.
Will utilize problem solving and motivational interviewing to help motivate and engage participants in the change process, ensuring the supervision and case management approach is culturally competent, gender-responsive, trauma-informed, and adheres to evidence-based risk-need-responsivity principles.
Help ensure we maintain a safe working environment by reporting any hazards or unsafe conditions, e.g., liquid spill in hallway, to the appropriate facilities staff member or supervisor.
What we are looking for:
Master's degree in Social Work, Mental Health Counseling, Psychology or other relevant clinical discipline.
One to two years of experience providing clinical services to individuals to families or groups managing challenges in Behavioral Health and the Criminal Legal System.
Strong interpersonal skills and emotional intelligence.
Must be able to navigate 5 boroughs of NYC and perform field work in a variety of weather conditions.
Must be able to lift 5-10 lbs.
Ability to work effectively within a diverse team environment.
Must be able to sit for extended periods.
Additional, preferred skills we are looking for but are not required:
Active licensure in a clinical discipline in New York State, preferred.
Experience providing direct clinical services in a field-based setting strongly preferred.
