Essential Functions:
Provide excellent and consistent customer service and support to applicants, the client, constituents, and program team members.
Assist applicants with the completion and submission of their program applications, as needed.
Review submitted applications for completeness and ensure that the program has received all documentation and information needed to perform an eligibility review.
Review applicant vulnerability factors and assign appropriate priority status to their application.
Conduct an orientation and introductory call to assigned applicants and request any application documentation or information needed to make the application complete.
Ensures program applicants are continuously updated regarding the status of their program application. Frequent, diligent, and professional communication required.
Obtains a working knowledge of applicant needs and program eligibility criteria.
Understands program requirements and other key objectives.
Understands program processes from start to finish and communicates those processes clearly to applicants.
Gathers applicant documentation and uploads to program system of record.
Records all communications in the program system of record.
Position is required in office and you will be required to travel to several intake centers in order to collaborate directly with clients, casemanagement and leadership regarding program applications.
Qualifications:
A Bilingual (Spanish/English) CaseManager should possess 2 years of demonstrated experience in the qualifications identified below:
Ability to manage effectively with or without subordinates.
Knowledge, skills, and abilities necessary to perform the job function with little to no supervision, while remaining acutely aware of timelines, meeting deadlines, and performance measures.
Ability to acquire a working knowledge of applicable rules and regulations and the ability to provide technical assistance.
Excellent written and oral communication skills, strong analytical skills, ability to work independently, and effective interpersonal skills.
Intermediate level Microsoft Office skills; knowledge of creating tables and graphs in Microsoft Excel; ability to quickly learn new software applications.
Detail-oriented with close attention to program compliance requirements, record keeping guidelines, and file closeout expectations.
Strong customer service skills and knowledge of customer service best practices.
Ability to maintain the confidentiality of program information.

The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a CaseManager 3 in an Early Intervention Unit. DDD provides individuals with developmental disabilities, and their families, services and supports that are flexible, high-quality, and member-driven. These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community.
Applicants for this position cannot be licensed or certified by the DDD Office of Licensing, Certification and Regulation, cannot have a principal interest in or have a spouse or close family member with a principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor.
Job Duties:
Essential Duties and Responsibilities include, but are not limited to:
In compliance with requirements (including mandated timelines), drive vehicle to conduct home visits and meet with individuals/families to develop relationships and become familiar with family dynamics/priorities/resources/ concerns/supports and make appropriate referrals to community agencies.
Convene and facilitate multi-disciplinary Individualized Family Service Plan (IFSP) meetings (a minimum of every 6 months for every child) to support children with developmental disabilities and their families to work toward successful acquisition of outcomes within the child's natural environment.
Offer the family (at the IFSP) the AzEIP surveys; educate parents of their rights and responsibilities and procedural safeguards. Teach parent advocacy skills. Assist families with the grieving process when applicable. Collaborate directly with the Department Of Child Safety (DCS) in meeting a child's needs while in the DCS system. Assess each child's progress on a regular basis.
Knowledge, Skills & Abilities (KSAs):
Knowledge in:
Methods, procedures, and techniques used in the development of Individualized Family Service Plans.
Health Insurance Portability and Accountability Act (HIPAA).
Federal and State statutes and agency policies and procedures relating to the care and assistance of individuals with developmental disabilities.

A minimum of a Bachelor's degree in Early Childhood, Early Child Special Education, Family Studies, OR a Bachelor's degree in a closely related field (e.g., Psychology, Social Work, Elementary Education, or Sociology) with review of course and approval by ADES/ AzEIP; OR if hired in the AzEIP system as a Service Coordinator before July 1 2001, documentation of continued satisfactory work in the AzEIP system in a Service Coordinator position from that date of hire.
If this position requires driving or the use of a vehicle as an essential function of the job to conduct State business, then the following requirements apply: Driver's License Requirements. Any employee operating a motor vehicle on State business must possess and retain a current, valid class-appropriate driver's license, complete all required training, and successfully pass all necessary driver's license record checks. Graduate of an accredited nursing program; Associate's degree or higher preferred
Current RN license in the state of practice or valid multistate compact license

Knowledge of hospice regulations and Medicare Conditions of Participation preferred

As RN CaseManager provides continuity of care, care coordination, identifies patient/family goals, develops a Plan of Care with patient, family, IDT, attending and physician, partnering facility and evaluates progress towards goals. The Plan of Care will be updated according to policy/guidelines and as needs change.
Following a patient's death, assists with closure contact and support in collaboration with IDT.
Directs and supervises the care provided by the LPN and Home Health Aide following agency policy. Evaluates services provided by the Home Health Aide every two weeks with periodic on-site supervisory visits.
Demonstrates the ability to administer, monitor and document medications as prescribed by attending physician. Uses age-specific care practices and provide appropriate patient/family teaching.

How you will contribute
You will determine donor eligibility; to include, proper management of informed consent, AIDS education and confidential self-exclusion, medical history interview and acceptance requirements, physical examination, blood testing, and reviewing Serological Protein Electrophoresis (SPE).
You will evaluate donor reaction(s), which occurs at the facility as outlined in the SOPs. Follow applicable SOPs for medical emergencies including the development of Center Physician's standing orders and donor transport to emergency care facilities.
You will refer to the Center Physician or Medical Affairs (as applicable) when in need of providing unacceptable findings to donors, or guidance concerning medical or technical issues, including donor safety and eligibility.
You will support the Hepatitis B and Seasonal Flu vaccination programs for employees as applicable.
You will manage employee incidents and determine whether further evaluation is required by occupational health/ER. Refer to EHS guidance regarding employee incidents.
You can be a Pandemic Coordinator when authorized by EHS and support investigations associated with pandemic threats within the local community as indicated by EHS or Medical Affairs.

Conduct administrative duties - Attend medical conferences and courses for ongoing care and education. Assess patient care plans and identify areas of improvement to ensure quality of care.
Requirements:
Bachelor's degree and a license to practice nursing in the state required
A minimum of six months of recent medical experience
Must be able to stand or sit for long periods of time
Must be able to push, pull, reach, and bend frequently
Must be able to lift up to 50 pounds
