QUALIFICATIONS: Bachelor's Degree Required in psychology, social work, education or a related field. REPORTING SUPERVISOR: IPS/FOWARDS Manager
GENERAL DUTIES: Provide one on one or group instruction, training, and supervision for youth and adults to help them become active and productive members of their community with a greater sense of inclusion, confidence and life satisfaction. Assist participants to achieve greater levels of personal, social, emotional, and leisure independence. Be an integral part of the team during the implementation and documentation of goals and objectives per each individual's treatment plan.

SPECIFIC DUTIES:
*Plan, adapt and implement activities, which enhance the personal growth, community inclusion, social awareness, emotional regulation, and vocational skills of each participant and ensure that the participant is an active partner in determining their plan/activities.
*Schedule, facilitate, and oversee activities that promote independence such as peer interaction, financial literacy, development of daily living skills, health and wellness, recreational and leisure skills, community mobility, career exploration and awareness of community resources.
*Transport and accompany individuals to activities as needed in personal and company vehicles.
*Operate and utilize agency software needed to carry out job responsibilities while maintaining security and ensuring confidentiality.
*Develop and structure ācurriculumā based on the acquisition of daily living skills, participant preferences, goals, and fostering independence.
*Complete 100% time reporting on a daily basis to maximize reimbursements for services. Appropriately document all services and supports being provided, in alignment with the treatment goals and objectives, and communicate such with referring agency. Proficiently communicate with family, providers and clinical team when necessary.
*Provide travel training on city bus routes and investigate other transportation options available for participants.
*Promote and enforce safety as well as model proper social conduct in all settings in which services are being rendered: homes, businesses, places of employment, and the general community. Provide personal assistance for individuals, as needed, in order to provide a safe and clean person and environment. This may include but is not limited to assistance in the restroom, and clean up.
Offer guidance, support, and encouragement to cultivate positive and healthy development of the participant and goal completion.
Perform additional duties as assigned by Supervisors.
