As the Physical Therapist Assistant (PTA), you will perform treatments and provide exercise instruction and patient education aimed at improving and enhancing the patient's well-being, while following the established plan of care.
**Primary Responsibilities:**
+ Responsible for following all state specific laws governing the provision of physical therapy in home care, to follow the treatment set only as defined by the supervising PT; to perform only those procedures that he/she is qualified and trained to perform and to make all recommendations for changes in the plan of care to the supervising Physical Therapist
+ Instructs and aids patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training
+ Observes, records, and reports to the supervising PT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition. Coordinates care with the other members of the healthcare team as appropriate
+ Instructs patient, family, caregiver, and other members of the health care team in the areas of therapy within the scope of the Physical Therapy Assistant
+ Visit patients within the service area

**Required Qualifications:**
+ Licensed, registered, or certified to assist in physical therapy in state of practice.

$49,500 - $109,300 annual total cash target pay
$28.56 - $63.06 per visit point
$23.8 - $52.55 hourly rate
Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of "direct" and "indirect" patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable. Qualifications

Demonstrated ability to retrieve and enter information using various proprietary software applications and create/modify documents and complex spreadsheets using Microsoft Office suite.
Bachelor's degree with coursework in math, engineering or related areas (or equivalent) and at least 5 to 7 years of directly related large property or Highly Protected Risk (HPR) experience in property loss prevention or risk engineering.

The Senior Ultrasound Clinical Account Specialist (ULS CAS) is accountable to provide technical and clinical product support to health care providers including Electrophysiology (EP) physicians, Interventional Cardiologists (IC) and laboratory staff for electrophysiology diagnostic and therapeutic catheters and systems - with an emphasis on the AcuNav and SOUNDSTAR catheters - to enable practitioners to perform advanced cardiac ablation and interventional procedures respectively.
The CAS is responsible to build and maintain mutually beneficial relationships with physicians & EP laboratory staff in order to identify and foster interest in new products or product indications to grow and develop the company's product and technology utilization. As the Senior Ultrasound Clinical Account Specialist, you will:
* Provide expert clinical product and technical assistance and training to EP/IC physicians and staff on the effective use of AcuNav and SOUNDSTAR catheters in EP and IC procedures.
* Provide expert clinical product and technical assistance and training to physicians and EP Lab Staff on the effective use of electrophysiology systems and catheter equipment (e.g., The CARTO System and appropriate software modules including CARTOSOUND , CARTOMERGE and the Stockert RF generator) during case procedures within an assigned geography, and in a manner that leads to meeting and exceeding business goals.
* Educate customers on all electrophysiology products to optimize effective usage by providing technical and clinical information and in-service training. Shares best practices to increase value for customers.
* Use consultative selling techniques to identify potential sales opportunities within the account.
* Create awareness of electrophysiology solutions and facilitates Territory Sales Manager (TM) contact with the key decision makers to drive incremental business.
* Maximize customer case support capability through proper planning and scheduling techniques.
* Drive collaboration and maintains consistent, open lines of communication across the assigned responsibilities with internal partners including regional teams/Pods (i.e. TMs, EP CAS and other ULS CAS), as well as support personnel (i.e. Marketing, FSE, RBD).

As the **Hospice Nurse Practitioner** , you will provide consultation in palliative care, symptom management and supportive care to meet the needs of the Hospice patient as requested by the referring physician.
**Primary Responsibilities:**
+ Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and reports clinical findings to the Hospice Medical Director
+ Consults with the Hospice Medical Director as needed, informs primary physician of recommended services, and collaborates with other physicians as needed
+ Initiates and coordinates the plan of care. Documents problems, appropriate goals, interventions, and patient/family response to hospice care. Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care

**Required Qualifications:**

Compensation for this specialty generally ranges from $104,500 to $156,000. * Develop, implement and monitor customized Special Servicing Instructions (SSI's) to guide the property risk engineering services being provided on specified accounts.
* Develop comprehensive Account Reviews for the Underwriter, in accordance with Engineering and Underwriting guidelines, in order to influence positive underwriting decisions and outcomes.
* Develop and present comprehensive Engineering Service Stewardship reports which serve as a platform to partner and relationship build with key external stakeholders.
Qualifications
* Advanced knowledge, skills and experience in a specialized field, property risk assessment, or property risk analysis.
* Fully effective interpersonal, writing and other communication skills required to develop and maintain relationships with customers, peers, and industry contacts.
* Demonstrated ability to retrieve and enter information using various proprietary software applications and create/modify documents and complex spreadsheets using Microsoft Office suite.
* Bachelor's degree with coursework in math, engineering or related areas (or equivalent) and at least 5 to 7 years of directly related large property or Highly Protected Risk (HPR) experience in property loss prevention or risk engineering.
