JOB SUMMARY
The Licensed Clinical Social Worker (LCSW) provides clinical assessment/problem identification, individual, family and group therapy and case management. Develops and monitors treatment plans, monitors treatment progress, documents significant interactions, and maintains clinical charts and other reporting data. Must be licensed or provisionally licensed and requires a Master's degree in a mental health field. Reported to the Department Lead.
The Kid Clinic is a school-based pediatric clinic that serves children ages 2 weeks to 21 years old and offers counseling services for children 4 years old to 21 years old. The Kid Clinic is a collaborative effort between Campbell County Health, Campbell County Medical Group, and Campbell County School District to provide primary medical care, behavioral health, and substance use counseling services in Northeastern Wyoming.
ESSENTIAL FUNCTIONS
Provides assessment, diagnosis, and treatment of behavioral health clients.
Provides clinically appropriate therapy utilizing best practices whenever possible.
Is actively engaged in the department, solves problems, participates in strategic planning, and contributes positively to workplace moral.
As assigned by manager and location participates in on call crisis rotation after hours and assists with walk-in crises during business hours.
Works positively and collaboratively with courts, law enforcement, physicians, hospital staff, Department of Family Services, Probation Offices, Campbell County School District, and other community agencies as applicable.
Maintains clinical documentation in a manner as required by department policy, the Standards for the Operation of Community Mental Health and Substance Abuse Programs, and the Medicaid Policies and Procedures Manual, and in accordance, as applicable with SAMHSA TIPS and TAPS.
Maintains competence in multicultural issues and demonstrates respect and understanding for people from diverse cultural backgrounds.
Adheres to established policies and procedures for Behavioral Health services or Kid Clinic and Campbell County Memorial Hospital.
Actively engages in departmental quality assurance and utilization management activities and processes to include monthly chart reviews.
Provides community-based psychotherapy to individuals, groups, and families as applicable.
Participates in activities to promote mental health and/or the prevention of substance abuse, particularly among children and adolescents, as appropriate.
Must be free from governmental sanctions involving health care and/or financial practices.
Complies with the hospital's Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures.
Demonstrates working knowledge of state and federal laws pertaining to HIPPA, CFR 42 and 45, EMTALA etc.

Summary
This position provides direct casework services to residents and is responsible for overseeing a resident caseload. It also tracks and documents resident progress in case reports while working to meet all required internal and external contract standards and policies.
Primary Duties and Responsibilities
Creates, updates and maintains case records on each assigned resident thoroughly and in a timely manner.
May be required to conduct assessments using objective tests, and completes intake/assessment forms, referral documents and observable behavior;
Collects resident history of criminal background, family dynamics, domestic environment, and substance abuse. Identifies clinical issues such as history of violence, escape, medical and prioritizes which issues are appropriately addressed during treatment process.
Evaluates and addresses any urgent medical or dental condition at the time of intake.

Conducts ongoing assessments, interventions, individual and group lectures and reports resident treatment progress.
Teaches residents to budget money in responsible manner.

Completes monthly reports as required to track resident progress as well as discharge summaries.
Develops continuing care treatment plans/strategies for residents prior to discharge as well as aftercare services.

Minimum Requirements

Clinical Social Worker Health Care Facility Surveyor - Wyoming (#1287) CMS's Long-Term Care Basic Training and SMQT certification are required Impact Recruiting Solutions is currently seeking a Clinical Social Worker Health Care Facility Surveyor to fill an opening with a Quality Improvement Consulting Company and will work in a technically exciting environment supporting internal and external customers nationwide.
Requirements of the Clinical Social Worker Health Care Facility Surveyor:
Must have a Master's degree in Social Work
Must have successfully completed CMS's Long-Term Care Basic Training and passed the Surveyor Minimum Qualifications Test (SMQT).
Must have at least two (2) years of recent experience working as a surveyor accrediting or certifying facilities that serve the residents of long-term care facilities.
Must maintain current licensure to practice as an LCSW
Demonstrated history of independent decision-making skills to direct and effectively manage the survey process.
Ability to set priorities independently and collectively in performing survey tasks.
Ability to openly discuss conflicts/controversy, and to seek assistance when appropriate to make decisions and resolve conflicts.
U.S. Citizenship is required for this position
Ability to travel up to 75% of the time on a regular basis is required

Responsibilities
Enhabit Home Health & Hospice is looking for a Licensed Clinical Social Worker (LCSW) to provide professional, comprehensive, family-oriented services to individuals in the home setting.
* Assess patients' and families' psychosocial, environmental, and financial needs.
* Formulate, implement, and evaluates a plan of care in collaboration with patient, family, and other caregivers, and provides case management as appropriate.
* Assist the team in understanding the social and emotional factors related to the patients' health problems.
* Maintain documentation in patient's record per internal regulatory and professional standards.
* Monitor, observe, and evaluate changes and progress in patient's condition and environment. Report changes, progress, or lack of progress to physician and/or nurse case manager.
* Acts as key source in patient situations such as: ineffective patient/family coping and decision making advance directives long term or assisted living placement substance abuse abuse/neglect and/or bereavement.
Qualifications
* Must be a Licensed Clinical Social Worker (LCSW) in the state of employment.
* Must possess one year of social work experience in a health care setting.
* Medicare home health or hospice experience is preferred.
* Must be capable of making independent decisions relevant to client's needs.
* Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.
