Overview:
Provides non-medical casemanagement services and medical casemanagement services under the Indiana State Department of Health, including social service assessments and connecting HIV+ individuals with needed services offered by Federal, State, and Local community programs. QUALIFICATIONS

Duties/Responsibilities:
Develops, delivers/implements, revises, and evaluates individualized patient-centered care plans under the supervision of a physician
Provides casemanagement to an assigned group of patients and ancillary staff
Understands and submits required data collection sets (OASIS, PAR, etc.) and other documentation as requested by the state and/or, federal government and insurance payers within required timeframes
Makes the initial evaluation visit and regularly reevaluates the patient's nursing needs per state and payor requirements
Acts as an advocate for patient welfare and coordinates care between patients, their families/caregivers and/or their authorized representative, the agency and other healthcare providers/facilities/ outside agencies

Submits documentation within 24 hours or per state requirements
Maintains confidentiality of patient, employee, and agency matters
Takes on-call duty nights, weekends, and holidays, as assigned. When functioning as the on-call RN acts as the after-hours supervising nurse and makes nursing assessment and triage decisions under the guidance of the overall Clinical Supervisor as applicable
Immediately reports to Nursing Supervisor any patient incidents/variances or complaints
Demonstrates competent performance of technical skills according to established procedures
Maintains acceptable attendance status, per Agency policy
Reports all incomplete work assignments to Nursing Supervisor
Demonstrates effective time management skills through daily documentation and infrequent overtime for routine assignments
Ensures that the following information/instruction is given in writing to the patient/caregiver/representative: visit schedule with frequency of visits, complete medication profile, any treatments to be administered by agency staff (POC), any other pertinent instruction related to the patient's care needs, name and contact information of the agency's ClinicalManager

Provide monthly Progress Reports by due date to supervisor to referring agencies and Kid Traks (if applicable) as well as any other specific written or verbal information requested by referring agencies.
Provide Treatment Plans, Safety Plans, and other required clinical documents/resources/tools within 7-30 days of referral to referral agencies. Updated Treatment Plans are due no less than every 90 days to supervisor for review and sent to referral source by pre-arranged due dates.
If applicable, complete visitation/parenting time logs and send to referral sources within 48 hours of services rendered ensuring all required sections of log are compliance with current state content standards.
Oversee completion of program required assessments, including but not limited to, PFS-2, NCFAS, UCLA PTSD, IARCA packets upon admission, discharge, three month and six-month follow-up.
Record all billable/direct and non-billable/indirect time on a daily basis, utilizing the established database while ensuring an accurate timesheet is maintained.
Document use of and adherence to evidence-based models and treatment for all services requiring use of a model that has been approved by the state of Indiana and GJR.
Obtain and submit requests for court appearances to Compliance Coordinator monthly by deadline established.
Submit Attendance and Compliance sheets for clients on caseload to Compliance Coordinator and supervisor by monthly deadline.
Appear at court hearings, case conferences, CFTM's or staffing as necessary or requested by referral source or supervisor.
Use strength-based approaches and develop protective factors for clients including but not limited to developing parenting skills and child development knowledge, improving social and emotional competence, accessing concrete supports, building social connections, and improving resilience.

How you'll contribute
A Intensive Outpatient Program Therapist (IOP) who excels in this role:
Conducts individual and group therapy sessions to educate patients regarding psychological, emotional, or substance abuse problems
Displays active involvement in treatment planning process
Provides family session counseling to all patients to ensure appropriate communication and involvement of family members and support groups
Actively communicates with clients, families, and outside referral sources
Works well with the interdisciplinary team including physicians, utilization review and nursing staff members
Supports UR team functions and provide treatment updates or documentation for authorizations.
Completes appropriate paperwork for clients in a timely, accurate, and complete manner
Completes required assessment paperwork including patient assessment scales, Columbia, psychosocial/LOC assessments (not applicable at all facilities), psychosocial/LOC updates, and assigned assessment paperwork
Demonstrates appropriate crisis intervention and de-escalation skills
Acts as member of team when a crisis occurs, assisting others to ensure patient safety
Enacts the code system per company policy and training
Completes accurate assessments of patients utilizing clinical skills
Assists care coordination department as requested

What we're looking for
Applicants should have a master's degree in social work or counseling. Additional requirements include:
Prior experience with psychiatric and chemical dependency patients.
Current unencumbered clinical license per state of practice guidelines.
CPR and CPI certified within 30 days of employment.
May be required to work flexible hours.
