Primary Duties

Stays well informed and frequently communicates with program manager and customers / employers to ensure quality work is being performed by participant.
Meet with program mentor to review participant progress and potential barriers.
Encourage and facilitate integration of the participant with co-workers to build professional and appropriate relationships.
Take notes on participant performance and problems. Record IPP/IEP related data and ensure that IPP/EPP objectives are accomplished.
Serve as a member of the participant's IPP/IEP Team. Evaluate participants using assessment form and gather baseline data for development of IPP/IEP's.
Perform other responsibilities as assigned.
Qualifications: Education, Experience, and Certification(s)
High school diploma or General Educational Development (GED) required
Valid driver's license and/or access to reliable transportation to perform work-related travel. Eligible drivers must have a good motor vehicle record (MVR)
Previous experience supporting adults with disabilities preferred.
Flexibility to work outside of normal business hours, including evenings and weekends is highly preferred.
Pass and maintain Department of Human Services background clearance is required.
Knowledge, Skills, and Abilities

Cleans cold food area, serving tables, refrigerated units, and equipment in cold food area.
Responsibilities:
* Has knowledge of soaps/chemicals used on a daily basis. Understands proper use.
* Sets up cafeteria serving areas and salad bar with all required foods and beverages, utensils and other supplies. Appropriately stores desserts after cold food set-up is complete
* When assigned to cafeteria, sets up hot food side and cold food side correctly for service. Serves hot and cold foods as requested and according to portion control list. Serves customers in a timely manner. Resupplies salad bar, condiments, and crackers, soda, milk, juices, etc. as needed.
* Correctly checks inpatient nursing units for soda and ice cream par levels. Correctly assembles non-patient supplies. Delivers supplies to non-patient areas. Has supply sheets initialed. Properly stores patient supplies in cooler.
* Prepares, delivers and/or sets up and serves all food items required for special functions and coffee breaks.
* Reports all equipment malfunctions to supervisors.

Maintains open and consistent communication with the Property Manager, Leasing Consultants, and the Maintenance team.
Assist with move-in/move-out process (inspections, distributing key packets and literature, collecting rental payments and /or missing paperwork, etc.) as directly by management.
Available during non-business hours to respond to lockouts and other minor service issues. *Compensation provided.
Other Requirements
While performing the duties of this job, the employee will need to travel from floor to floor and will need to travel outside between buildings in varying outdoor weather conditions. Assist with move-in/move-out process (inspections, distributing key packets and literature, collecting rental payments and /or missing paperwork, etc.) as directly by management.
Available during non-business hours to respond to lockouts and other minor service issues. *Compensation provided.

While performing the duties of this job, the employee will need to travel from floor to floor. JOB SUMMARY:
Assists the Therapeutic Recreation Coordinator in carrying out individual and group activities to meet the psycho social needs of each resident.
ESSENTIAL FUNCTIONS:
Treats all information about residents, their condition, and family as confidential information.
Complies with established Corporate and Departmental policies and procedures, and maintains established standards and practices.
Helps develop, coordinate and assist residents to programs and activities.
Develops and guides Reality Orientation activities (daily announcement, current events, large calendars, visual aides, seasonal decorations,...)
Schedules regular use of Homewood's van to include scenic drives, shopping trips, transportation to community events, etc.
Assists in assigning volunteers to specific job responsibilities in activities, making sure the volunteer has adequate materials and supervision.
Participates in resident care planning when pertinent information can be helpful.
Facilitates in special functions from outside groups and special activities on weekends and evenings.
Documents and charts attendance of all activities and reports observed changes in resident's physical and emotional status to the licensed nurse.
Performs other functions as directed by the supervisor.
QUALIFICATIONS:
Ability to read, write and understand English well.
Has compassion, understanding and empathy for older persons.
Ability to understand and follow oral and written instructions.
Ability to listen and to communicate to residents at a level they can understand.
Ability to and enjoys encouraging and motivating older persons.
Qualities of creativity, resourcefulness and imagination in planning resident activities.
High school diploma or equivalent preferred.
Ability to drive Homewood's van and has a valid driver's license.
Experience working with elderly preferred.
PHYSICAL REQUIREMENTS:
Must be able to constantly work on their feet during an 8 hour shift. Sometimes involved in assisting with resident transfers or repositioning. Must be able to react quickly and decisively in emergency situations and during unexpected behavior of residents. Must be able to push residents who are in a wheel chair frequently throughout an 8 hour shift (and be able to drive the company van.)
