The Role Itself
Conducts bio-psychosocial assessments under supervision.

Overview
Summary: Under general supervision, is responsible for professional case management work of routine difficulty, and performs related work as required.
Distinguishing Features: This is the trainee/entry level class in the DCS Case Manager job series. An employee in this class learns to perform a variety of case management duties for children under State supervision, in State custody, or at risk of State custody, and their families. This class differs from DCS Case Manager 2* in that an incumbent of the latter is fully trained and functions at the working level.
* An applicant appointed to this flexibly staffed class will be reclassified to the next higher class in the series after successful completion of a mandatory one-year training period; inadequate or marginal performance during the training period will result in automatic demotion or termination.

Lutheran Social Services of Central Ohio is currently seeking a Case Manager for Support Services for Veteran Families (SSVF) for Faith Mission in Columbus, Ohio. Faith Mission is one of the largest homeless shelters in Columbus, Ohio, providing meals and shelter to men, women and Veterans with dignity and compassion.

What will I do as the SSVF Case Manager with Lutheran Social Services?
Work closely with community and government agencies (social services, shelters, faith and community based organizations, prisons, jails, VA facilities, VA grant and per diem programs, legal aid, settlement houses, landlords, etc) to identify very low-income veteran families who are at risk of homelessness.
Using appropriate tools, screen identified families for eligibility, prioritize those families in greatest need and assess whether other interventions may better serve the veteran family being screened.
Perform intake for new participants, including needs assessment, and mental/physical health screenings.
Refer ineligible families to alternate resources/supports as available in the community or through the VA.
Work closely with clients to develop and implement participant specific, individualized, intentional action-oriented goal plans based on the CTI model, with a focus on achievement of housing stability.
Implement planning systems that reflect client strengths and needs, client responsibilities, Case Manager responsibilities, and action steps to help clients in achieving their goals.
Provide budgeting and other planning assistance toward the goal of housing retention.
Coordinate with VA services, veteran programs, Faith Mission Resource Specialists and other community resources to assist clients in seeking income, employment and/or other needed supports.
Link clients to internal resources as appropriate for financial supportive services in the form of direct client assistance payments to third party (e.g., landlords).
Refer clients to partner agencies and community resources that benefit their short-and long-term goals.
Interface closely with all Faith Mission staff, VA staff and public assistance agencies to provide quality wrap-around services and access to benefits for clients.
Gather information on potential resources for clients in the larger community, creating working relationships and disseminating information for immediate and ongoing support to client.
Maintain clear and precise data and case notes, including timely submission of any CQI reports and careful documentation of all client interactions.
Actively participate in weekly Case Review, working with colleagues toward positive client outcomes.
Work with client to maintain on-going support relationships with community resources, family and friends.
Successfully complete the Critical Time Intervention training.
Contribute to an atmosphere of safety, sanctuary, dignity, support and resource through adherence to the Faith Mission Code of Ethics and Confidentiality Agreements.
Assist with crisis intervention and de-escalation by providing leadership, and by modeling professional and calm problem-solving behavior for clients, colleagues and the community.
Requirements for the SSVF Case Manager with Lutheran Social Services:
4 year degree in Social Work or related field OR equivalent experience required.

Mental Health Provider Goal: It is the mission and purpose of Quality Correctional Care to assist corrections-involved individuals in their healthcare journey from initial interaction with law enforcement through successful transition back to the community by providing counseling and treatment.
Mental Health Provider Duties/Responsibilities:

What you will do as Case Manager: The Case Manager (QMHS) provides professional mental health and case management supportive services to youth ages 8-17 and their families on an outreach basis, in the home, school, and community. Job Description
Primary responsibility of a Case Manager is coordinating all aspects of client care, support, and case plans to assist clients in obtaining/maintaining housing stability, preventing recidivism of homelessness, and improving overall independent living skills and quality of life. The Case Manager will be responsible for:
Providing support services to individuals and families that are active in a Homefull Shelter, Housing, or Prevention program (prevention, shelter rapid rehousing, permanent supportive housing);
Working with assigned caseload on developing and working toward individualized service plans (ISPs, aka Case Plans) that include housing stability in addition to other goals related to stabilizing a household's housing, food access, and income;
Completing high quality clinical documentation at the time of services, as part of Collaborative Documentation efforts when possible;
Maintaining client case records according to agency policies and procedures and HMIS policies and procedures and responding to any identified deficiencies on quality assurance chart reviews;
Contributing to performance outcomes including frequency of client contacts, provision of mental health support, housing stability, increasing income, obtaining/maintaining benefits, and recidivism;
Conducting the above work via home visits, meeting clients at community locations as needed, transporting clients to appointments, meeting in offices for site-based programs, and conducting telehealth as consented to by clients and appropriate for client need;
Submitting reports and responding to information requests as needed;
Meeting general productivity and CPST productivity goals based on assigned caseload;
Participating in ongoing education and training to develop professional skills, achieve QMHS status via Homefull training, and maintain licensure (if applicable);
Completing CPST documentation and billing spreadsheets following guidelines;
Maintaining high standard of client care, contributing to an environment where all clients are treated with dignity and respect;
Maintaining Homefull's code of conduct and follow all confidentiality guidelines;
Valuing and demonstrating a commitment to diversity, equity, and inclusion;

What this job entails:
Managers lead shifts every week, making sure customers get a fast, accurate, friendly experience every visit. Social Worker for Housing Case Manager, Regional Areas - Central Ohio
Columbus, OH
Classification: Full-Time, Non-Exempt, Monday-Friday, 8:30 am-5:00 p.m. Office located in Delaware, OH.
Salary: $19.10 per hour + a fantastic benefit package*
Job Focus: This position will help households quickly access appropriate services to address housing crises, increasing exits to permanent housing, decreasing the length of time clients are homeless, and reducing returns to homelessness. This position will also assist people who are literally homeless or are at risk with homelessness by assisting with securing and maintaining stable housing.

Qualifications
An understanding of and passion for The Salvation Army's Mission
Licensed social worker, preferred. But willing to train if no license.
Associate degree or higher in Social Work or related field, preferred, willing to train the right candidate.
Experience working with persons facing housing crisis and/or homelessness, preferred or we are willing to train the right candidate.
Must be ablet o demonstrate excellent organizational skills and the ability to work with a diverse population.
