Job Duties and Responsibilities
Conduct regular outreach in the community to identify and build relationships with individuals experiencing homelessness.
Engage with clients in a compassionate and respectful manner to assess their needs, build trust and encourage participation in services.
Conduct assessment of needs to identify barriers to housing stability and prioritize client needs.
Develop individualized service plans and goals in collaboration with clients to address behavioral health, healthcare, employment, housing and other essential needs.
Connect clients with appropriate resources and services, including emergency shelter, transitional housing, healthcare providers, mental health and substance use treatment and social services.
Advocate for clients to ensure access to benefits and community-based supports.
Provide crisis intervention and supports to clients experiencing mental health crises, substance use issues, or other emergencies.
Collaborate with crisis response teams, law enforcement, emergency medical services as needed to ensure client safety and well-being.
Build partnerships and collaborate with community organizations, homeless shelters, healthcare providers, law enforcement, and local government agencies to coordinate services and support for individuals experiencing homelessness.
Participate in community meetings, task forces and committees focused on homelessness prevention and solutions.
Maintain accurate and up to date documentation of client interactions, assessments, service plans, referral and outcomes in accordance with agency policies and regulatory requirements.
Prepare reports and data analysis to track program outcomes, measure impact and support funding applications.

The Housing Outreach Specialist is responsible for developing and implementing the health plans housing strategy and programs. The Role
As an Intake Specialist, you'll play a key role in our operations and customer success by managing new patient referrals and coordinating timely start-of-care visits. SCOPE OF ROLE
Reporting to the Manager, Residential Services, the Specialist, Residential Care provides direct care support services to clients in a residential facility. Working closely with the House Coordinator, you will operationally support the facility, supervise the residents in the completion of their daily tasks and handle any emerging situations that require immediate attention. What You Will Be Doing
Teach independent living skills and assist the youth and their family in achieving stated objectives.
Connect and work cooperatively with family members, significant others, and community partners for a successful outcome for our youth.
Assist in the development of individual's comprehensive services plans, transition plans and follow up plans.
Review, sign and implement ISP's as needed.
Supervise all activities of the independent living center during assigned shifts.
Participate in apartment tours and oversee the maintenance of the facility.
Develop and implement specific activities and tasks for our clients related to the objectives in their ISP with appropriate guidance.

Under the supervision of their Site Director, the Residential Youth Specialist provides full range of support services to youth in foster care while performing a limited assessment of youths needs and comprehensive treatment planning and service delivery in collaboration with children, county caseworkers, other providers as deemed necessary. The Residential Youth Specialist is responsible for support and adherence to the mission of Unks Place, Inc., enforcing rules, modeling positive behaviors, developing life skills, and organizing events related to school, health, wellbeing and recreation.
EDUCATION/EXPERIENCE
Must have at least a high school diploma or GED, with at least one year of experience providing direct service to youth;
Or an Associates Degree in Human Services related field and one year or more of providing direct service to youth;
Or a bachelors Degree in an approved human services field (preferred)
PRIMARY DUTIES AND RESPONSIBILITES
Facilitate resident medical and other outside appointments, including but not limited to communicating appointments to residents and appropriate staff, and transporting residents to their appointments.
Administer First Aid to resident in response to minor medical problem as appropriate.
Prepare and supervise meals for residents.
Ensure that all physical and hygiene needs are met for residents.
Process and submit critical incident reports by THE END OF EACH SHIFT.
Communicate with local law enforcement, mental health providers, county caseworkers and all other entities when appropriate in a respectful manner.
Conduct room checks and wellness checks, 3 times within hour.
Conduct random room searches for potential contraband or anything that does not belong.
Facilitate Intake for new incoming youth.
Guide and instruct residents in proper daily living life skills; Supervise and participate in the completion of daily chores.
Comply with licensing standards, certification standards, employee handbook, client rights, and any other rules of conduct.
Provide transportation for youth to appointments as needed or assigned. Drives agency vehicle in safe and lawful manner.
Keep accurate and timely records and writes thorough and objective reports including, but not limited to, log entries, critical incident reports, phone calls, visits, progress reports, enrichment groups, and mental health services.
Create a family atmosphere that promotes unity, leadership, teamwork, responsibility, and positive reinforcement, and health relationships.
Provide a positive role model by maintaining appropriate dress, language, social skills, and hygiene.
Establish boundaries. Respond to youths needs with fairness, objectivity, firmness, care and concern. Observe behavior and emotional climate within the group and with each individual youth and respond appropriately.
Maintain familiarity with all emergency procedures relative to hazard such as: Tornadoes, power outages, floods or snow storms.
