Summary:
Codes and abstracts hospital medical records (including Inpatients, Observation, Outpatient Surgery, Invasive Outpatients, and Emergency Department) for diagnostic and procedural coding.

The
Coding Specialist
is responsible for coordinating and participating in the coding of pertinent medical information from a variety of complex records and coding edits to include diagnosis, treatment of illness and procedures performed while ensuring accuracy of work adherence to established coding procedures of ICD-CM (International Classification of Diseases) and CPT-4 (Current Procedural Terminology).

Review and educate physicians, advanced nurse practitioners and other staff as necessary on documentation requirements and coding guidelines.
Communicates to Coding management any claims edit updates needed, identified during concurrent audit of billing and coding.
Review claim denials submitted to Coding by RCM for changes to coding and updates to system edits to prevent future denials. Troubleshoot denials for cause and determine resolution.
Research and review third party payer guidance for establishment of system edits.
Participating in company processes for obtaining facility medical records (view with direct access, request via online forms, send provider requests) as well as being the organization representative to provide other departments with records from certain sites for which coder has access.
Maintain facility EMR system access as assigned (complete demographic forms, attend facility training, etc).
Participates in audit, education, and coding team meetings to discuss solutions to coding and documentation scenarios identified during coder assessment of provider coding and billing selections.
Facilitate new provider system access and training in charge capture systems.
Timely and appropriate coding of services as required to meet production needs - Meet or exceed required departmental productivity standards on a consistent basis.
Performs a variety of other Coding Compliance duties as needed.
Maintain strict confidentiality in accordance with HIPAA regulations and Company policy.

The Release of Information Specialist supports secure and authorized exchange of protected healthinformation at Charlie Health. This role will be responsible for ensuring Charlie Health complies with all state and federal privacy laws while providing access to care documentation.

Responsibilities
Maintains confidentiality and security with all protected information.
Receives and processes requests for patient healthinformation in accordance with company, state, and federal guidelines.
Ensures seamless and secure access of protected healthinformation.
Establishes proficiency in HealthInformation Management (HIM) electronic document management (EDM) systems.
Answers calls to the medical records department and responds to voice messages.
Retrieves electronic communication, faxes, opening postal mail, and data entry.
Responds to internal requests via email, slack, or any other communication platform.
Documents inquiries in the requests for information log and track steps of the process through completion.
Determines validity from documentation provided on authorizations, subpoenas, depositions, affidavits, power attorney directives, short term disability insurance, workers compensation, health care providers, disability determination services, state protective services, regulatory oversight agencies and any other sources.
Sends invalid request notifications as needed.
Retrieves correct patient information from the electronic medical record (EMR) and other record sources.
Verifies correct patient information and dates of services on all documents before releasing.
Provides records in the requested format.
Acts in an informative role within the organization regarding general release of information questions and assists with developmental training.
Documents accounting of disclosures not requiring patient authorization.
Scans or uploads documents and correspondence in EMR.
Communicates feedback, new ideas, fluctuating volumes, difficulties, or concerns to the HIM Director.
Participates in teams to advance operations, initiatives, and performance improvement.
Assists with other administrative duties or responsibilities as evident or required.
Requirements
Associates Degree required or equivalent in release of information experience.
1 year experience in a behavioral health medical records department, or related fields.
Experience in a healthcare setting is highly desirable.
Experienced use of email, phones, fax, copiers, MS office, and other business applications.
Ability to prioritize multiple tasks and respond to requests in a fast-paced environment.
Ability to maintain strict confidentiality.
Extreme attention to detail as it relates to accurate information for medical records.
Professional verbal and written communication skills in the English language.
Work authorized in the United States and native or bilingual English proficiency
Familiarity with and willingness to use cloud-based communication software-Google Suite, Slack, Zoom, Dropbox, Salesforce-in addition to EMR and survey software on a daily basis.
Please note that members of this team who live within 45 minutes of a Charlie Health office are expected to adhere to a hybrid work schedule.

The total target base compensation for this role will be between $44,000 and $60,000 per year at the commencement of employment. Please note, pay will be determined on an individualized basis and will be impacted by location, experience, expertise, internal pay equity, and other relevant business considerations. Further, cash compensation is only part of the total compensation package, which, depending on the position, may include stock options and other Charlie Health-sponsored benefits.

Responsible for maintaining the professional reimbursement program. Ensure compliance with current payments and rules that impact billing and collection.
Duties and responsibilities
The Medical Billing Coder performs billing and computer functions, including patient & third party billing, data entry and posting encounters. Typical duties include but are not limited to:
Follow-up of any outstanding A/R all-payers, self-pay, and the resolution of denials
Prepares and submits clean claims to various insurance companies either electronically or by paper.
Handle the follow-up of outstanding A/R all-payers, including self-pay and /or the resolution of denials.
Answers question from patients, FHCHC staff and insurance companies.
Identifies and resolves patient billing complaints.
Prepares reviews and send patient statements and manage correspondence.
Handle all correspondence related to insurance or patient account, contacting insurance carriers, patients and other facilities as needed to get the maximum payments and accounts and identify issues or changes to achieve client profitability.
Take call from patients and insurance companies regarding billing and statement questions.
Process and post all patient and/or insurance payments.
Reviewing clinical documentation and provide coding support to clinical staff as needed.
Qualifications
High School diploma or GED with experience in medical billing is required.
A certified professional coding certificate (CPC AAPC), knowledge of third party billing requirements, ICD and CPT codes, and billing practices are also required.
Excellent interpersonal and communication skills and ability to work as a member of the team to serve the patients is essential.
Must be detail oriented and have the ability to work independently.
Bi-lingual in English and Spanish highly preferred.
FQHC/EPIC experience is desirable.

Minimum Qualifications:

Ability to research and analyze data, draw conclusions, and resolve issues; read, interpret, and apply policies, procedures, laws, and regulations.
