Key Responsibilities:

Job Title & Role Description:
The Care Specialist - ECM is responsible for coordinating care for high-complexity patients, mainly working in the field to provide chronic care coordination and support. This role involves direct outreach to patients through phone calls, home visits, and community interactions. The Care Specialist primarily works in patients' homes and communities (90% of the time) and engages in virtual or telephonic support (10% of the time) The Care Specialist will assess patient needs, help set health goals, and ensure that patients receive the appropriate care and resources, with a focus on increasing access to preventative care, reducing emergency room visits, and enhancing self-management. The role requires excellent communication skills, critical thinking, and the ability to work independently and adapt to evolving challenges.
Skills Required:
At least 2 years of relevant work experience as a Community Health Worker, Peer Support Specialist, Medical Assistant, or in a similar role.
High school diploma or GED required.
A valid drivers license and auto liability insurance.
Reliable transportation and the ability to travel within assigned territory or as needed.
Experience in care coordination for individuals with chronic conditions, behavioral health conditions, or with patients experiencing housing insecurities including homelessness.
Strong interpersonal and motivational interviewing skills to build trust and rapport with patients.
Familiarity with trauma-informed care, care coordination, and patient education.
Proficiency in the use of electronic medical records (EMR) systems and basic computer skills.
Technologically savvy and able to manage documentation and data entry effectively.

Multi-lingual capabilities preferred but not required.
Prior home care or Enhanced Care Management experience a plus.

Requirements
2+ years of prospect research, proposal development, and managing/growing the portfolio of government, foundation, and corporate grants.
2+ years of proven track record of fundraising success with event and program sponsorships
2+ years of proven track record of fundraising success with individual giving campaigns.
Eager to learn new platforms-HubSpot experience is a bonus, but not required
Proficiency in project management and office software and tools (e.g., Basecamp, Microsoft Office)
Strong interpersonal and communication skills (verbal and written), including the ability to build and nurture relationships with donors, board members, and staff.
Bachelor's degree in Business Admin, Nonprofit Management, Public Admin, Marketing and Communications or similar equivalent professional and/or certificated experience.
Ability to manage multiple projects simultaneously and meet deadlines in a fast-paced environment.

Physical & Travel Requirements:
This is an onsite position in San Rafael office with minimal travel to the Santa Rosa office. Standard mileage reimbursements apply.
Ability to work occasional nights and weekends to support events/special projects.
Must have visual acuity; be able to operate a computer and other office equipment; be able to move about inside an office; frequently communicate with others in person, on the phone or via the computer; occasionally lift or move items up to 25 lbs.

Compensation:
The budgeted starting salary that CVNL reasonably expects to pay for this position is $68640 to $73000. This takes into account the wide range of factors that are considered in making compensation decisions including but not limited to experience, skills, knowledge, abilities, education, licensure and certifications, analysis of internal equity, external market comparisons for similar positions in similar geographic locations and size. Offers are determined based on final candidate qualifications and experience.

Berkeley Rep is seeking a detail-oriented and highly organized Development Coordinator to join our Development (fundraising) team. This position supports Berkeley Rep's efforts to attract and engage its donors through relationship management, with a particular focus on donor prospect research and donor engagement. This position is well suited to someone interested in gaining or expanding their experience in non-profit fundraising and administration. The position's essential job duties must be conducted in person on Berkeley Rep's campuses and cannot be performed remotely.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Donor Prospect Research, Tracking, and Reporting (35%):
Support Berkeley Rep's major individual donor giving program by:
Conducting donor prospect research
Tracking donor prospects in Tessitura, Berkeley Rep's CRM patron database
Tracking trustee recruitment efforts
Taking meeting minutes and tracking action items
Maintaining information on donor prospects and efforts to engage with them
Supporting the Development team's reporting and analysis needs by generating reports and lists
Donor Stewardship (35%):
Maintain accurate donor recognition across platforms (signage, print programs, website, named spaces)
Coordinate benefits fulfillment for major donors, including invitations to rehearsals and other behind-the-scenes opportunities
Help to streamline and support benefits fulfillment for all donor giving levels
Coordinate the use of Berkeley Rep's two donor lounges: order and maintain stock of food, drinks, and paper goods; provide guidelines and training to staff regarding room set up, service, and schedule; track costs; schedule event bookings
Provide ticketing assistance to trustees and major donors during peak times
Engage with trustees and donors by phone, email, and at events
Event Support (25%):
Provide event planning support, including managing RSVPs, attendee research, and creation of collateral materials
Provide day-of-event support including set-up, management, and execution of event day tasks
Provide logistical support for domestic and international donor theatre trips
General Administrative Support (5%):
Provide administrative support for the Director of Development and the Development team, including but not limited to: scheduling meetings, note-taking and action items, and expense reports
Provide grant-writing and institutional funder prospecting support at peak times
Assist with data entry and acknowledgements during peak times

Requirements
Qualifications and Skills:
One to two years of experience in non-profit development, administration, or customer service
Exceptional attention to detail and organizational skills
Proficiency with MS Office, including Excel
CRM/database experience a plus, particularly Tessitura
Ability to provide exceptional customer service to the Theatre's valued supporters
Strong written and verbal communication skills
Proven ability to prioritize and manage projects and deadlines effectively
Resourcefulness and strong problem-solving skills
Ability and interest in creating efficient systems and processes
Sensitivity to confidential information and demonstrated diplomacy
Must be comfortable and thrive in fast-paced, diverse group environments.
