Job Role Summary
The Care Coordinator provides skills training and development to clients as required by medical necessity. In collaboration with the treatment team, the Care Coordinator conducts necessary client assessments as well as formulates, develops and revises treatment plans. The Care Coordinator advocates for clients to ensure access to needed services which include developing community relationships and resources.
Essential Functions and Responsibilities
• Embraces, understands, and operates under the Recovery Model, including AIDET (Acknowledge, Introduce, Duration, Explanation, Thank You) and the spirit of motivational interviewing
• Provides skills training and development to clients in group and individual settings as guided by medical necessity outlined in the person-centered treatment plan
• In collaboration with the treatment team, the Care Coordinator conducts necessary client assessments as well as revises treatment plans
• Provides crisis management and emergency interventions including but not limited to cardiopulmonary resuscitation and approved area-specific crisis intervention techniques
• Reports to treatment team in staffing including but not limited to successes and challenges, assessment of client progress, barriers to success, any level of care changes, etc.
• Meets the agency's standard of providing the appropriate service at the right time, right place, and right frequency as determined by the client's level of need
• Meets the agency's standard of timely completion of all documentation including interpreting and applying service and billing procedures according to client needs as well as completing billing according to guidelines for timeliness and accuracy
• Establishes and maintains a safe environment at all times through on-going assessment of milieu/appointment environment and intervenes as indicated in person-centered treatment plan as well as by nursing or physician order
• Provides case management including working with family, significant others and community supports to assist clients in meeting goals
• Meets agency requirements for continuing education
• May provide transportation in company provided vehicle and/or personal vehicle as directed by treatment team and guided by medical necessity; maintains a copy of Eskenazi Health's vehicle-related accident reporting procedures in vehicle

• Bachelor's degree required
o If incumbent holds a bachelor's of social work degree, incumbent must also possess a current, unencumbered Licensed Bachelor's Social Worker license in the State of Indiana

Track completion of documentation and process updates resulting from local and global procedural changes, audits, complaint investigations, documentation reviews, product reviews, process reviews and equipment reviews
Develop effective and productive working relationships with colleagues at CM sites
Lead multi-functional/cross-site/cross-company project teams as needed
Provide backup, as appropriate, for the Manager - CMO Operations
Basic Qualifications:
Bachelor's Degree required in a STEM / technical field
2 or more years manufacturing experience preferred
Demonstrated high degree of ownership / accountability
Strong communication (written and oral) skills, teamwork, and influence
Solid technical writing skills
Ability to organize and prioritize multiple tasks

• Review customer experience measures for trends and execute improvement recommendations as necessary across impacted programs
• Collaborate with the vendor to ensure adherence to training and quality standards, while proactively identifying and reporting instances of non-compliance and report deviations.

Responsibilities
* Leads group onboarding trainings for new hires on their first day in the facility, including standard training videos and assessments for safety, quality control and warehouse equipment.
* Shadows new associates during their first two-four weeks to assess their performance and provide on-the-job guidance. Retrains associates based on identified areas for improvement and performance metrics.
* Based on the training needs of the facility identified by site leadership, cross-trains staff so that associates possess the skills to work in each department/work area of the facility.
* When not training staff, completes daily responsibilities of an experienced associate in one of the facility's departments, such as receiving or picking.
* Conducts individual trainings with new hires that do not possess past experience with certain equipment in the warehouse, such as forklifts. Ensures that associates achieve sufficient proficiency with equipment prior to beginning job duties.
* Provides focused, on-the-job training for specific departments or teams to improve daily functioning, implement operational changes, and other initiatives as identified by site leadership.
* Regularly communicates with Supervisors and Managers to understand training needs and to discuss retraining and cross-training of specific teams or individual associates.
* Ensures that onboarding documentation is properly completed for new hires by coordinating with a Human Resources or Administrative colleague in the facility.
* Participates in facility EH&S teams or committees in order to understand all safety procedures and ensures updates to policies and procedures in the training content (translations may be needed).

What You'll Do:
A successful Vehicle Operations Specialist (VOS) provides administrative support to ensure every customer transaction request, received via phone, mail, or email, is properly documented within our current tasking system, and manage the timely assignment of requests to the appropriate transactional queue. The VOS will review the received request and/or documents to validate request is complete enough to provide to a transaction team for fulfillment.
