WHAT WE'RE LOOKING FOR:
The MDS Coordinator (LPN, RN) is responsible for overseeing the resident assessment and care planning process and ensuring compliance with federal and state regulations related to resident assessments, quality of care and Medicare/Medicaid reimbursement.
Key Responsibilities
Conduct and complete the Minimum Data Set (MDS) assessment to evaluate residents' physical, psychological and functional status, including the implementation of Care Area Assessments (CAA)s and triggers.
Evaluate each resident's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
Prepare and electronically transmit timely reports to the national Medicare and Medicaid databases.
Develop a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
Provide information to residents/families on Medicare/Medicaid and other financial assistance programs available to the residents.
Ensure that MDS notes are informative and descriptive of the services provided and of the residents' response to the service.
Assist with completing the care plan portion of the residents' discharge plan. Evaluate and implement recommendations from established committees as they pertain to the assessment and/or care plan functions of the health campus.
Qualifications
Must have and maintain a current, valid state LPN or RN license
Three (3) to five (5) years' experience working in the MDS or assessment role in a senior residential care, healthcare, senior living industry or long-term care environment, preferred
Current, valid CPR certification required
Compensation will be determined based on the relevant license or certification held, as well as the candidate's years of experience.

POSITION SUMMARY
The Clinical Support Nurse - Infection Prevention and Employee Health is responsible for assisting the organization's infection prevention and employee health program, including, but not limited to surveillance and reporting, compliance with regulatory and accreditation agencies, staff education and training, and employee health and wellness activities for new and existing staff. This position and its associated responsibilities are dynamic and may involve a variety of projects based on current organizational needs.
ESSENTIAL JOB KNOWLEDGE
In collaboration with company leadership, promotes adherence with infection prevention and employee health policies and procedures.
Supports completion of employee health requirements in compliance with applicable regulatory bodies (e.g., CMS, OSHA, The Joint Commission, etc.)
Assists the department's Program Manager in ensuring the following activities are completed: new hire assessments; return to work guidelines; reasonable accommodations; vaccine programs; PPE, hand hygiene, exposure control plans; infection prevention and control guidelines; wellness programs and education.
Supports and acts as back-up to the department's Program Manager regarding worker's compensation matters.
Assists management with regular Infection Control risk assessments, mitigation, and response efforts through events and education, as appropriate.
Maintains current sources of evidence-based data, guidelines, and regulations related to infection control and employee health issues.
Provides consultation regarding patient and employee infection control concerns.
In collaboration with leadership, monitors, investigates and reports suspected or confirmed outbreaks of infectious diseases, health of employees, and community-based infections.
Performs contact tracing for employees or patients, as needed.
Supports the company's community-based programs, including but not limited to, in-home testing and vaccination programs.
Assists leadership team with clinical support across the organization.
Initiates, promotes, collaborates with departments in providing regular employee wellness programming and education including remote and in-person options.
Provides CPR/BLS certification for UWHCD employees.
Maintains professional and technical job knowledge.
Successfully completes initial and ongoing organizational and department education, training, and competency requirements.
Infection Control: Understands, demonstrates, and follows organizational infection control guidelines (CDIC).
Quality: Focuses on improving processes and champions efficiency in daily work activities.
Safety: Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization's occurrence reporting system and participating in developing improved safety processes, as appropriate. (ADM015, ADM016, OPS044, Mission Statement).

JOB REQUIREMENTS
Licenses:
State of Wisconsin Driver's License.
State of Wisconsin Professional licensure for Registered Nurse.
Education:
Associate degree in nursing required.
Bachelor's degree in nursing preferred.
Experience:
Three or more years of nursing experience in a health care setting required.
Previous occupational health experience strongly desired.
Previous recent experience and strong knowledge base in Infection control and prevention.
Miscellaneous:
Excellent interpersonal skills; written and verbal communication, and organizational skills.

Job Description

Collaborates with support services to provide the patient with a safe, clean, and secure environment.
Implements clinical and technical aspects of care in accordance with policies and procedures; intervenes with proper techniques, procedure, and safety precautions to meet the individual needs of the patient.
Recognizes emergency situations and is familiar with emergency equipment.
Responsible for room assignments daily regarding case assignments, equipment, instrumentation, supplies and case carts.
Demonstrates the knowledge and skills necessary to provide the care appropriate to the age of the patients' services on the assigned unit.
Demonstrates knowledge of the principles of growth and development over the life span and possesses the ability to assess data reflective of the patient status. Includes being able to interpret the appropriate information needed to identify each patient's requirements relative to the patient's age-specific needs.
Able to perform all duties of a Staff Nurse.
