The MedicalRecordsClerk completes analysis and filing of all medicalrecords and reports in a timely and accurate manner. Duties and Responsibilities
Regular responsibilities include but are not limited to the following:
Completes clerical duties including answering phones, responding to emails and processing patient information
Compiles statistical reports and assists in maintaining logs as required by hospital policy or state and federal regulations
Completes birth certificates
Retrieves records for physicians and other hospital staff
Ensure patient charts, paperwork and reports are accurate and completed in a timely manner

Job Description
Candidate will be working the front desk in the Health Information Management Department (HIM). They will be transferring calls, customer service, data entry and assisting with HIM/MedicalRecords. Testing will be required for the job. (Grammar/Spelling, Data Entry, Computer Skills)
• Demonstrates competence in the areas of critical thinking, interpersonal relationships, and technical skills.
• Demonstrates ability to provide care/service safely and efficiently for the care of each patient. NA
• May be responsible for one or more of the tasks below:
• Retrieves, reviews, edits, and completes data entry of dictation completed by staffand resident physicians with an average of 30-35 dictations processed per hour.
• Validates that electronic medicalrecord documents are interfaced into the correct system.
• Maintains prioritized, optimal workflow through cooperative work distribution (i.e., timely processing and distribution of documented reports, while maintaining the departmental goal of processing key reports within 24 hours).
• Follows departmental procedures when making corrections and processes 20-23 transcribed documents/hour.
• Assists physicians as needed with instructions on dictation and completing reports and deficiencies.
• Contacts physicians as needed for incomplete documents.
• Handles customers efficiently and courteously.
• Responsible for a high standard of quality and quantity.
• May assist with monthly physician statistical reporting and data correction.
• Submits worksheet to supervisor daily.

Essential Functions:
Ensures receipt of all records of patients discharged from the hospital by reconciling with list of discharged patients or records associated to clinic and/or hospital patients.
Scans completed hospital and clinic records into the electronic medicalrecord, as required.
Analyzes the chart, confirming all scanned documents have been scanned into the correct location, with the correct, date, time, and subject, per the naming conventions, and makes corrections/rescans, as needed.
Purges and records and/or scans old records prior to destruction.
Non-Essential Functions:
Transcribes dictation from physicians and other professional staff accurately and timely (48 hours or less), coordinating, as needed, with any outsource transcriptionist.
Tracks completed transcription.
Reviews for completeness of medicalrecords of patients treated in the hospital inpatient and outpatient settings to facilitate timely coding, including for presence of history and physical within 24 hours, discharge summary, and progress notes, and prepares and provides outstanding documentation lists by provider for completion.
Searches for any additional diagnoses in archived records, other encounter problem lists, or other encounter documentation, providing to coder/HIM Coordinator to consider any additional coding to abstract.
Facilitates response to any Discern Notifications.
Facilitates internal and external queries, as needed.
Ticklers external queries and follows up weekly until response or until queries have been communicated in creative ways on at least three occasions.
Verifies charge corrections have been made, and releases encounters only awaiting charge correction.
Forwards any encounters with missing treatment authorizations to Registration supervisor.
Coordinates Specialty Clinic schedules, ensuring that required documentation from the clinics are received in time for coder to code the encounter.
Handles requests and inquiries for patient information, dispersing the information with accountability to all regulatory entities and according to the facility's policies and procedures.
Reviews for completeness of release of information requests.
Compiles data from medicalrecords for periodic and statistical reports.
Maintains registers for statistical compilation (acute, skilled, intermediate, outpatient, emergency, death, transfer).
Evaluates existing forms and their processes, recommending and implementing improvements to enhance efficiency and user experience.
Designs, develops, and updates electronic and paper forms based on user requirements, ensuring clarity, efficiency, and consistency.
Collaborates with various departments to gather requirements, solicits feedback, and ensure forms meet their specific needs and comply with organizational policies.
Coordinates interdisciplinary forms committee meetings for review and standardization of new, updated, and deleted forms across departments.
Maintains central forms database, ensuring all forms are properly version-controlled, updated, and accessible.

Summary:
Registration Specialists provide superb customer service to patients by managing the patient flow, providing information to family members and visitors, and ensuring accurate collection of patient demographic and insurance information. This position will provide educational literature to patients, obtain required signatures, and request upfront payment for services based on estimated patient responsibility. Ensure patients are directed to the appropriate departments.

Responsibilities:
Essential Job Duties and Responsibilities:
1. Gathers and accurately enters patient demographic and insurance information into the hospital's Electronic Health Record (EHR), obtain signatures on required documentation such as treatment consent forms, ABN's, financial responsibility letters, etc.
2. Thorough understanding of health insurance coordination of benefits and the importance of prior authorizations. Completes follow-up calls with insurance companies, doctors, and other departments as needed to ensure accurate patient registration. Contact patients after services are rendered to obtain any missing information.
3. Ensure patient identification, insurance cards, forms, and other critical documents are scanned and accurately indexed into the EHR. Ensure the correct data is entered into the EHR by verifying from the patient documentation. Place detailed notes on patient accounts for all activities.
4. Maintain current knowledge of the EHR by attending regular training classes, reading training documentation in a timely manner, and attending monthly team meetings.
5. Understand patient status in order to accurately assign a patient to inpatient, observation, ambulatory, or CDU rooms, coordinating this directly with the nursing supervisor or other nursing staff.
6. Use clear language to education patients and families about Conditions of Admissions, Patient Rights and Responsibilities, Notice of Privacy, Important Message from Medicare/Tricare, Notice of Admission Status, Patient Inquiry Release form, and all other applicable forms. Offers patient/family hospital literature explaining these forms. Maintains a high level of patient confidentiality at all times. Both within the facility as well as outside the facility.
7. Utilizes appropriate protective apparel and devices when performing admitting duties in the patient's room or the Emergency Department. Knows the protocol following a blood/body fluid exposure incident.
8. Create estimates for services to patients and prospective patients. Discuss payment options with patients and request the estimate patient out-of-pocket cost prior to the service. Ensure accurate cash handling and balance a cash drawer on a daily basis.
9. Provide excellent customer services including answering phones, directing patients to their destination, answering basic account questions, and maintaining a clean and inviting environment. Be professional and courteous.
Hours will be dependent on patient census and workload. Ability and willingness to work a flexible schedule, to include after-hours and weekends as necessary.
