Position Summary
Under the supervision of department leadership, performs a second level review of records and data to ensure all processes are performed in accordance with standard operating procedures and all regulatory and accrediting standards. Assists in developing and maintaining documentation required for compliance, operations, training, quality, process improvement and/or environmental health and safety program. Partners with departmental management in collecting and analyzing data to support continuous improvement resulting in value-added customer/donor service and increased product yields and financial results while maintaining compliance and quality.

Responsibilities
Uses data and information collected through medicalrecord review to assess organ donor potential, to identify missed opportunities for donation, and to evaluate the effectiveness of referral processes, thereby supporting continuous improvement efforts and organizational growth.
Maintains confidentiality while reviewing OPO/TB records to ensure compliance with organizational procedures and regulatory and accrediting standards.
Interprets and prepares performance and compliance reports for donor hospitals, medical examiners, and tissue processors.
Identifies and develops relationships with hospital partners' key health information management staff
Ensures accurate and timely data collection, data entry, and data analysis related to medicalrecord review, donor potential, and regulatory reporting requirements
Prepares metric reports according to organizational standards for structure, style, format, order, clarity, etc., while using professional judgement within set parameters with regards to overall design and data presentation.
Submits required regulatory reports to appropriate agency by required timeframe.
Performs audits of operational functions.
Practices a high degree of autonomy in a self-directed manner, demonstrating continuous improvement, innovation, and creativity in problem solving, sound critical analysis and judgment
Generates the appropriate deviation reporting forms and communicates with departmental management.
Supports external inspections and facilitate timely audit responses.

Qualifications
Education
Bachelor's Degree required
Degree in a Biological Science preferred
Equivalent combination of education and related experience (3-5 years) may be substituted for the degree with HR approval required
Experience

Experience in data analysis, record review, or quality control preferred

VITAL RECORDSCLERK Register of Deeds - Courthouse $16.70 to $25.05 Per Hour Job Summary: The Vital RecordsClerk is a custodian of all vital records (Birth, Death, Marriage, Divorce, and Domestic Partnership) responsible for processing vital records requests and registering Marriages/Deaths into the State SVRIS System in accordance with state statutes and directives.
Job Duties and Responsibilities:
Process customer requests efficiently in person, online (Vitalchek), or by mail per State Statutes. Process Federal, State, County, and City entities requests. Calculate/verify fees given to cashier. The volume of documents sold is approximately 6,300/month.
Perform data entry functions to cashier nine (9) categories of vital records fees. Receive tender (approx. $2,000 daily) via cash, check, and credit/debit card. Close out Kiosks daily. Balance cash drawer and prepare cash count slip. Troubleshoot balancing issues.
Assist customers with the process for fixing problems on their vital records per State Statutes. Verify information of each record requested from State Vital Records Office (SVRO) required to update data for Statewide Issuance. Verify requests from Social Security office.
Analyze original vital records (deaths, marriages, domestic partnerships, and other transaction records) to determine if information is accurate. Correct the records by collaborating with all entities (officiants, funeral directors, State Vital Records employees, County employees, etc.).
Index, file, and refile all vital records accurately. Scan each document for printing on security bond paper. Verify both indexing and scanning processes for accuracy. Teach genealogists how to use indexing system and provide help with their family searches
Perform all duties as may be assigned.
Minimum Requirements:
Current Resident of the State of Wisconsin
High School Diploma or GED equivalent
At least one (1) year of recent customer service experience performing clerical work, cashiering, and data entry.
Must possess minimum typing/keyboarding standards with minimal errors at the time of application as well as cash handling aptitude. Pre-employment testing may be done during the interview phase to ensure these proficiencies.
Experience with general office equipment including personal computer, scanner, typewriter, calculators, embossers and printers.
At least basic experience with Microsoft Office, Word, Excel, and Outlook applications
The ability to lift/carry/push/pull up to 30 pounds on a regular basis
Preferred Experience:

Job Description:
As a Patient Service Representative, you will be responsible for providing a warm welcome to all visitors and providing them with the necessary information and assistance they need. Understand cancellation policy and reiterate it to patients.
Print end of the day reports.
Maintain office/reception area cleanliness.
Check and reply to all office emails
Check collected payments against the report and check all credit card transactions.
Call patients with outstanding balances
In the event you are traveling to another clinic, you are required to perform all duties and responsibilities of your role including administrative tasks.

Position Summary
Under the supervision of department leadership, performs a second level review of records and data to ensure all processes are performed in accordance with standard operating procedures and all regulatory and accrediting standards. Assists in developing and maintaining documentation required for compliance, operations, training, quality, process improvement and/or environmental health and safety program. Partners with departmental management in collecting and analyzing data to support continuous improvement resulting in value-added customer/donor service and increased product yields and financial results while maintaining compliance and quality.

Responsibilities
Uses data and information collected through medicalrecord review to assess organ donor potential, to identify missed opportunities for donation, and to evaluate the effectiveness of referral processes, thereby supporting continuous improvement efforts and organizational growth.
Maintains confidentiality while reviewing OPO/TB records to ensure compliance with organizational procedures and regulatory and accrediting standards.
Interprets and prepares performance and compliance reports for donor hospitals, medical examiners, and tissue processors.
Identifies and develops relationships with hospital partners' key health information management staff
Ensures accurate and timely data collection, data entry, and data analysis related to medicalrecord review, donor potential, and regulatory reporting requirements
Prepares metric reports according to organizational standards for structure, style, format, order, clarity, etc., while using professional judgement within set parameters with regards to overall design and data presentation.
Submits required regulatory reports to appropriate agency by required timeframe.
Performs audits of operational functions.
Practices a high degree of autonomy in a self-directed manner, demonstrating continuous improvement, innovation, and creativity in problem solving, sound critical analysis and judgment
Generates the appropriate deviation reporting forms and communicates with departmental management.
Supports external inspections and facilitate timely audit responses.
Organizes and correlates in an established manner all paperwork associated in the review process for record retention purposes.
Assists in the implementation of federal requirements, Versiti directives, and standard operating procedures.
Works collaboratively with customers as needed to ensure timely submission of required donor information.

Qualifications
Education
Bachelor's Degree required
Degree in a Biological Science preferred
Equivalent combination of education and related experience (3-5 years) may be substituted for the degree with HR approval required
Experience
1-3 years experience in a regulated environment where change management and continual process improvement were required and successfully implemented required
Experience in data analysis, record review, or quality control preferred
Knowledge, Skills and Abilities
Excellent written and verbal communication skills.
Knowledge of medical terminology.
Demonstrated knowledge of current Good Manufacturing Processes.
Strong analytical skills and attention to detail.
Knowledge of and ability to apply quality management/process improvement tools including LEAN, root cause analysis, and use of statistics.

* Receive and process requests for patient health information in accordance with Company and Facility policies and procedures.
