Specific Job Duties:
1. Support individuals we serve by maintaining regular contact and doing Individual Family Service Plans (IFSP) and Individual Program Plans (IPP).
2. Participate in Interdisciplinary Team (IDT) staffings and attend meetings in the community as needed including Individual Education Plan (IEP) and day program meetings.
3. Provide information and referral to generic services and resources in the community, including
a. Schools, recreational options and local support groups.
b. Supportive Employment/Habilitation funded day programs.
c. Local agencies (C.C.S., Dept. Rehabilitation, Social Security, Medi-Cal, IHSS).
d. Specialist physicians/ specialists (speech therapist, O.T./P.T.).
4. Coordinate and secure TCRC funded services, including:
a. Behavior services, training for independent living, supportive living.
b. Out-of-home placement, in home and out of home respite.
c. Infant and adult day programs.
d. Durable medical equipment and specialized services, non-Medi-Cal funded services.
Primary Job Functions:
1. Conduct strength-based assessments that communicate an understanding of the unique issues facing family members.
2. Involve individuals we serve and family in all aspects of person-centered planning and support activities.
3. Design, document, implement and monitor services and supports in a timely manner.
4. Facilitate planning team progress, identifying outcomes, needed services and supports.
5. Develop and maintain a network of vendors and community agencies.
6. Provide relevant information and training about developmental disabilities, services and resources.
7. Use effective communication and interpersonal skills that consistently respect children and adults with developmental disabilities and their families.
8. Demonstrate an operative knowledge of family/natural support and private/public resources and integrate these into service planning resulting in community-based outcomes.
9. Demonstrate an operative knowledge about developmental disabilities and how families are impacted.
10. Demonstrate an operative knowledge of a variety of approaches to intervention and support.
11. Demonstrate an operative knowledge of entitlement and benefit programs.
12. Demonstrate an operative knowledge of the Tri-Counties Regional Center system, the legal system, individual civil rights and overall advocacy.

ESSENTIAL FUNCTIONS
Coordinates the service needs of individuals on assigned caseload
Participates in the implementing and/or maintaining of IHSS services- including but not limited to: recruiting, scheduling, timekeeping and/or submission of timecards, report concerns to IHSS assigned social worker as needed.
Assists and train person served in advocating for their personal needs
Acts as liaison between Momentum WORK, Inc., and other agencies on behalf of people supported
Maintains the confidentiality of people served in all aspects of service provision
Provides teaching/learning opportunities and facilitates the acquisition of skills by using effective teaching/counseling principles and techniques
Train person served to access public transportation and assist in accessing benefits such as Social Security, SSI, IHSS and Section 8 housing.
Coordinates instruction and support to people served in acquiring and utilizing assistive technology/adaptive equipment and accessing modifications as needed
Coordinates activities of the DSP including assisting people served to train, direct, schedule, and maintain appropriate DSP to meet their personal support needs in all environments

Submits written weekly schedules to program management within the required timeframe and updates schedules as required when changes occur
Establishes and maintains effective working relationships with people served, individuals in their Circle of Support, family members, other agency staff including regional center and day program staff, employers, and relevant members of the community
Maintains a level of personal conduct in working with people served which will promote a friendly, comfortable, safe atmosphere where people served may progress socially, emotionally, and physically
Respects and always promotes the individual rights of people served including maintaining the dignity and privacy of each person

Projects for Assistance in Transition from Homelessness (PATH) is a U.S. federal grant program providing outreach and services for individuals with serious mental illness or co-occurring disorders who are homeless or at risk of homelessness. The PATH/HDAP Outreach Worker is responsible for providing assistance, trauma informed support to homeless, and formerly homeless residential clients with serious mental, substance use disorder and/or physical disabilities. The PATH/HDAP Outreach Worker is expected to identify homeless mentally ill adults in the community, assess their medical and psychiatric needs, and assist them in obtaining appropriate services, including but not limited to referrals to Behavioral Health, Drug and Alcohol, and the Department of Social Services (for Cash Aid/Food Stamps/MediCal and Social Security Disability application support). In addition to serving as the PATH Outreach Worker, this position also provides general support to all other HDAP programs (Housing & Disability Advocacy Program and to the Homeless Encampment Case Manager Program). The PATH/HDAP Outreach Worker will uphold and promote the program philosophy in all duties performed.
HOURS/ SALARY:
Part-time, non-exempt position. 24 hours per week. Anticipated hiring range is $25.50-$26.50. Responsibilities and Duties:
1.Identify and engage with homeless community members to assess needs and make appropriate referrals to supportive services.
2.Receive referrals from a variety of sources and meet with clients in a timely manner to assess their needs and appropriateness for services (includes completing intake paperwork with potential clients and following their progress until outreach services are no longer needed or desired).
3.Conduct VI-SPDAT surveys for determination of severity of needs and placement onto the Coordinated Entry Program for housing and support services.
4.Recognize and be available to clients in psychiatric crisis or emotional distress, linking them to appropriate support systems, including outpatient and crisis/inpatient treatment.
5.Function as a role model to clients with regard to rehabilitation and recovery, communication skills and work behavior.
6.Transport clients to and from appointments.
7.PATH: work with 25 unhoused, dual-diagnosis clients at a time for the PATH program, providing services and support as listed above.
8.HDAP: assist the HDAP Program Manager with 3-5 clients at a time who are actively pursuing Social Security Disability benefits (SSI, SSDI or a combination of both).
9.BHBH: assist the BHBH Housing Navigator with moving clients in and out of housing, which may also include furniture pick up, delivery and assembly, and provide general support to HHAP clients as needed.
10.HECM: work in partnership with the HECM case manager to ensure all PATH and HECM outreach clients are provided with a full range of services, which may include assisting HECM clients with independent living skills training, transportation, assistance with vocational/educational activities and housing stability, upon request.
11.Site management: Includes inspecting sites and assisting with cleaning and/or repairs.
12.Administrative Duties: Complete program paperwork in a timely fashion, including time sheets and financial receipts. Administrative duties may also include: maintaining individual paper and/or electronic client files; evaluating and charting client progress; developing service plans, admission and/or discharge paperwork; HMIS data entry, which includes entering and exiting all PATH clients; entering client notes within three business days of the client contact, adding entry/exit dates in HMIS as well as Excel and Word; meet or exceed State required contract outcomes.
13.Promote, encourage and maintain trauma-informed practices in all services and activities. Practice and promote, both individually and in team meetings, TMHA's Collaborative Communication Guidelines in order to facilitate clear, empathetic and person-centered communication.
14.Complete all assigned trainings.
15.Attend all HDAP, agency and other required staff meetings to coordinate case management activities with supervisors and program staff.
16.Must be comfortable working in a diverse and unpredictable environment and meeting clients “where they are at” with a “whatever it takes” non-judgmental, trauma-informed approach.
17.Assume other duties and responsibilities as assigned by the HDAP Program Manager and Program Director.

Projects for Assistance in Transition from Homelessness (PATH) is a U.S. federal grant program providing outreach and services for individuals with serious mental illness or co-occurring disorders who are homeless or at risk of homelessness. The PATH/HDAP Outreach Worker is responsible for providing assistance, trauma informed support to homeless, and formerly homeless residential clients with serious mental, substance use disorder and/or physical disabilities. The PATH/HDAP Outreach Worker is expected to identify homeless mentally ill adults in the community, assess their medical and psychiatric needs, and assist them in obtaining appropriate services, including but not limited to referrals to Behavioral Health, Drug and Alcohol, and the Department of Social Services (for Cash Aid/Food Stamps/MediCal and Social Security Disability application support). In addition to serving as the PATH Outreach Worker, this position also provides general support to all other HDAP programs (Housing & Disability Advocacy Program and to the Homeless Encampment Case Manager Program). The PATH/HDAP Outreach Worker will uphold and promote the program philosophy in all duties performed.
HOURS/ SALARY:
Part-time, non-exempt position. 24 hours per week. Anticipated hiring range is $25.50-$26.50. Responsibilities and Duties:
1. Identify and engage with homeless community members to assess needs and make appropriate referrals to supportive services.
2. Receive referrals from a variety of sources and meet with clients in a timely manner to assess their needs and appropriateness for services (includes completing intake paperwork with potential clients and following their progress until outreach services are no longer needed or desired).
3. Conduct VI-SPDAT surveys for determination of severity of needs and placement onto the Coordinated Entry Program for housing and support services.
4. Recognize and be available to clients in psychiatric crisis or emotional distress, linking them to appropriate support systems, including outpatient and crisis/inpatient treatment.
5. Function as a role model to clients with regard to rehabilitation and recovery, communication skills and work behavior.
6. Transport clients to and from appointments.
7. PATH: work with 25 unhoused, dual-diagnosis clients at a time for the PATH program, providing services and support as listed above.
8. HDAP: assist the HDAP Program Manager with 3-5 clients at a time who are actively pursuing Social Security Disability benefits (SSI, SSDI or a combination of both).
9. BHBH: assist the BHBH Housing Navigator with moving clients in and out of housing, which may also include furniture pick up, delivery and assembly, and provide general support to HHAP clients as needed.
10. HECM: work in partnership with the HECM case manager to ensure all PATH and HECM outreach clients are provided with a full range of services, which may include assisting HECM clients with independent living skills training, transportation, assistance with vocational/educational activities and housing stability, upon request.
11. Site management: Includes inspecting sites and assisting with cleaning and/or repairs.
12. Administrative Duties: Complete program paperwork in a timely fashion, including time sheets and financial receipts. Administrative duties may also include: maintaining individual paper and/or electronic client files; evaluating and charting client progress; developing service plans, admission and/or discharge paperwork; HMIS data entry, which includes entering and exiting all PATH clients; entering client notes within three business days of the client contact, adding entry/exit dates in HMIS as well as Excel and Word; meet or exceed State required contract outcomes.
13. Promote, encourage and maintain trauma-informed practices in all services and activities. Practice and promote, both individually and in team meetings, TMHA's Collaborative Communication Guidelines in order to facilitate clear, empathetic and person-centered communication.
14. Complete all assigned trainings.
15. Attend all HDAP, agency and other required staff meetings to coordinate case management activities with supervisors and program staff.
16. Must be comfortable working in a diverse and unpredictable environment and meeting clients “where they are at” with a “whatever it takes” non-judgmental, trauma-informed approach.
17. Assume other duties and responsibilities as assigned by the HDAP Program Manager and Program Director.

Arroyo Grande Care Center, a 99 bed skilled nursing facility, is in search of a part time Activities Assistant (life enrichment assistant). This is a fast paced job that requires you to be energetic, have the ability to work under pressure, be a people person, and multi task. You need to be able to work independently with little supervision, be flexible, be self-directed, and be dependable. You absolutely must be outgoing. BASIC FUNCTIONS
The primary responsibility of your job position is to assist the Activity Director in planning, organizing, developing, and directing the overall operation of the Activity Department following the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities.
PRINCIPLE DUTIES AND RESPONSIBILITIES
• Demonstrate a passion for caring for the elderly evidenced by interaction with co-workers, residents, families, and visitors
• Assist in developing, maintaining, evaluating, directing and providing daily activities for the residents
• Coordinate activities with other departments as necessary.
• Recommend to the Activity Director the equipment and supply needs of the activity department.
• Assist in-room residents by visiting with them, and perform assistance with crafts, projects, writing letters, as necessary etc.

EDUCATION/EXPERIENCE/JOB TRAINING
• Must possess, as a minimum, a high school diploma or its equivalent.
