Your Role:
Care Coordinator, Youth and Family - Heritage Behavioral Health Center (Decatur, Illinois)
As a Care Coordinator for Youth and Family, you will serve as the primary point of contact for youth enrolled in the Pathways to Success program and their families. Your mission is to ensure that each child receives integrated, person-centered care that addresses behavioral health, physical health, and social determinants of health. You will work collaboratively with families, providers, and community partners to develop and implement individualized care plans that promote stability, recovery, and long-term success.
Core Responsibilities:
Engage youth and families to build trust and ensure active participation in care planning.
Conduct comprehensive assessments to identify behavioral health needs, physical health concerns, and social service requirements.
Develop and maintain individualized care plans that reflect family voice and choice, cultural considerations, and strengths-based approaches.
Coordinate services across multiple systems, including mental health, primary care, education, child welfare, and juvenile justice.
Facilitate Child and Family Team (CFT) meetings to ensure collaborative decision-making and progress monitoring.
Monitor service delivery and outcomes, adjusting care plans as needed to meet evolving needs.
Provide crisis support and linkage to appropriate resources when urgent needs arise.

SUPERVISOR: Director of Equity and School Improvement and Building Principal(s)
JOB SUMMARY: The Mentoring coordinator is responsible for the overall workings of this exciting afterschool program. The coordinator is responsible for planning and preparation of the program, overseeing and hiring of the staff, supervising the daily operations and and supervision of students within the parameters established by funding and school district procedures. The coordinator must engage families, build relationships with students, be energetic, creative, and willing to work hours outside of the school day.
ESSENTIAL FUNCTIONS: To perform this job successfully, an individual must be able to perform each essential function satisfactorily.
Coordinate and implement grant-based after-school program in accordance with district/grant policies and regulations for the school.
Assist the Director of Equity in planning and designing the program and establishing and maintaining communication with stakeholders.
Engage mentoring staff to develop instructional and assessment practices linking the school day curriculum with the extended day program.
Work with staff to outreach and actively engage students and families in the program.
Facilitate the development and selection of Social Emotional Learning lessons and activities
Develop procedures and policies for operation of the afterschool program.
Supervise staff including planning staff schedules, reviewing timesheets, submitting requests for staff stipends, and arranging substitute coverage for absences.
Maintain student enrollment records, class schedules, and all other necessary documentation.
Meet regularly with extended day staff, facilitate orientation training, and staff development activities.
Work with community contractual partners (Urbana Park District, Prairie Center Health Systems, Adult Education, University of Illinois Cooperative Extension, Urbana School-Based Health Clinic) to plan and implement components of the project. Facilitate partner orientation.
Work with school district partners (Building Principal, Mentor/Volunteer Coordinator, Classroom Teachers, Social Worker, Family Liaison) to integrate components of the program.
Promote and market information about the after-school program to students, parents and the community.
Maintain and reconcile budget-related documentation and report information to the Director of Equity in a timely fashion.

QUALIFICATIONS:

Coordinators must be available for session hours and additional time as required to prepare and coordinate the program.
This position is not part of the collective bargaining unit.

Duties & Responsibilities
* Daily management of campus MHEAC programs and activities including but not limited to scheduling and attending meetings, gathering, drafting/editing, and distributing materials, and providing act/grant information as requested.
* Serve as an integral part of the MHEAC Act/grant coordination team with responsibility for developing strategies to effectively manage and enhance programs and activities funded by the grant.
* Serve as the MHEAC representative and campus liaison; coordinate outreach initiatives and promotion of the Act's mission and objectives, engage in the campus community and serve as an expert, advisor, and resource.
* Provide oversight of training and educational initiatives; plan, coordinate and facilitate training, educational programs, and presentations for internal and external training needs, and lead training programs to ensure related activities fulfill grant requirements.
* Gather data, draft, and finalize quarterly and annual reports for the Illinois Board of Higher Education.
* Research health and well-being grant funding opportunities; identify key stakeholders and coordinate efforts to develop strong grant funding applications.
* Coordinate outreach initiatives across digital engagement platforms such as SOS for Higher Ed: Suicide Prevention for Students and Togetherall; remain abreast of the Illinois App and other campus platforms including CampusWell.
* Research, interpret, and apply all relevant federal and state regulations and laws, University and sponsor policies, and sponsoring agency terms and conditions; advise University staff on how to best maintain compliance.
Additional Physical Demands
Standard office setting with some distractions such as phone calls, inter-office conversations, and visitors. While the work is primarily sedentary, some on-campus travel is required to attend meetings and training sessions, etc.
Minimum Qualifications
Education
* Bachelor's degree in public health, psychology, social work, higher education administration, public administration, or field related to the position.
Experience
* A total of two (2) years (24 months) in education, training and/or work experience in educational programming, grant administration, and/or project management.
* Demonstrated experience managing projects or grants that included budgeting and report preparation.

Essential Duties:
Manage the Appraisal Inbox and maintain an appraisal and evaluation order log.
Adhere to all applicable laws and regulations, including compliance with all appraisal and environmental rules and related regulations.
Assist the Appraisal Review Manager in the ordering of appraisal reports and draft appraisal engagement letters.
Provide updates to appraisers and Bank personnel on the status of assignments.
Communicate requirements effectively to external fee appraisers and external environmental engineering firms.
Perform administrative reviews of commercial and residential real estate appraisals reports for compliance with law and policy and for reasonableness and quality.
Maintain effective relationships with external fee appraisers and other vendors.
Conduct preliminary market research and gather property-specific data for appraisal and evaluation orders.
Perform real estate evaluations and validations.
Resolve valuation differences/discrepancies between appraisers and loan production staff while maintaining Appraiser Independence.
Maintain knowledge of real estate value trends and property developments to ensure valuations conform to current market conditions.
Update the Bank's Approved Appraiser list on a regular basis.
Remain up to date on regulations and knowledge surrounding the appraisal industry through continuing research, training, and development.
Education and Experience:
Associate's Degree in business, management, or a related field or equivalent experience
2+ years of experience in the following areas preferred:
Appraisal department or firm
Real estate lending (commercial and residential)
Real estate sales, broker, or investor
Equivalent combination of education and work-related experience
Work Schedule:
This position requires 40 hours each week with work primarily being done Monday-Friday between 8 a.m. and 5 p.m. Travel may be required as necessary to fulfil appraisal needs and working hours after normal business may happen on occasion.

As a Care Coordinator - Community Support Heritage Behavioral Health Center in Decatur, Illinois, you will provide person-centered care coordination ensuring individuals accessing behavioral health services receive seamless, comprehensive, and ongoing support within their assigned program area. This position is responsible for tracking clients from their initial engagement in services through successful connection to all recommended treatments, ensuring continuity of care.
